研究目的：

本研究试图找寻不同BMI值的非酒精性脂肪性肝病患者尤其是瘦人或非肥胖脂肪肝患者的差异性表达代谢物，探索外泌体circular RNA作为NAFLD尤其是瘦人或非肥胖NAFLD患者早期诊断的可行性及相关机制研究，为临床医生准确评估和预测不同BMI值NAFLD患者的病情进展程度，制定合理的诊疗方案及随访策略提供实验依据。  

Objectives of Study：

This study attempted to find the differential expression of metabolites in non-alcoholic fatty liver disease patients with different BMI values, especially lean or non-obese fatty liver patients, and explore the feasibility and related mechanism of exosomal Circular RNA as early diagnosis of NAFLD, especially in lean or non-obese NAFLD patients. It provides an experimental basis for clinicians to accurately evaluate and predict the disease progression of NAFLD patients with different BMI values and formulate reasonable diagnosis and treatment plans and follow-up strategies.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄16-75周岁，性别不限；
②每周酒精摄入：男性不超过210g，女性不超过70g；
③B超或CT检查存在脂肪肝的影像学改变；
④排除病毒性肝炎、药物性肝病、自身免疫性肝病及其他自身免疫性疾病、甲状腺疾病等其他导致脂肪肝的情况；
⑤既往无重大胃肠道手术史，如胃大部切除及胃肠吻合术、结肠癌根治术、小肠切除术等，但是阑尾切除术、消化性溃疡穿孔修补术等除外；
⑥无严重心、肺、脑、肾疾病及恶性肿瘤；
⑦女性排除妊娠或近期有受孕意愿者。

Inclusion criteria

① Age between 16 and 75, no gender limitation;
② Weekly alcohol intake: no more than 210g for men and 70g for women;
③ There were imaging changes of fatty liver in B ultrasound or CT examination;
④ Exclude viral hepatitis, drug-induced liver disease, autoimmune liver disease, other autoimmune diseases, thyroid diseases and other conditions leading to fatty liver;
(5) No previous major gastrointestinal surgery, such as gastrectomy and gastroenterostomy, radical resection of colon cancer, resection of small intestine, etc., except appendectomy and repair of peptic ulcer perforation;
⑥ No serious heart, lung, brain, kidney disease or malignant tumor;
⑦ Women were excluded from pregnancy or recent desire to conceive.

排除标准：

①合并有肝外纤维化疾病包括系统性红斑狼疮、风湿类疾病、肾功能衰竭、慢性阻塞性肺疾病、肿瘤类疾病、糖尿病、高血压病、冠心病等；
②排除合并甲、乙、丙、丁、戊型病毒性肝炎感染；酒精性肝病、自身免疫性肝病、胆汁淤积性肝病、药物性肝损害、遗传代谢性肝病以及晚期肝硬化合并肝脏肿瘤者；
③正在进行或既往接受过抗肝纤维化治疗或抗病毒治疗者，应用过降酶药物者；
④有黄疸，ALT 和/或AST 超过正常值上限2 倍及2 倍以上；
⑤与本研究相关的临床资料数据不完整的患者。

Exclusion criteria：

① Patients with extrahepatic fibrosis diseases including systemic lupus erythematosus, rheumatic diseases, renal failure, chronic obstructive pulmonary disease, tumor diseases, diabetes, hypertension, coronary heart disease, etc.;
② Ruled out the combination of a, B, C, D, E viral hepatitis infection; Alcoholic liver disease, autoimmune liver disease, cholestatic liver disease, drug-induced liver damage, genetic metabolic liver disease and advanced cirrhosis with liver tumor;
③ Those who are undergoing or have received anti-liver fibrosis therapy or antiviral therapy in the past, and those who have used lowering enzyme drugs;
(4) jaundice, ALT and/or AST more than 2 times or more than the upper limit of normal value;
⑤ Patients with incomplete clinical data related to this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Data recording: The investigator must ensure the completeness and accuracy of the data recorded. Data records must be clearly filled in with black ink. Correction of incorrect values and/or words is strictly prohibited. Use a single line to cross out errors, fill in the correct values/words next to them, and sign the modifier's initials and date of modification. 2. Data check: Quality control personnel or inspectors of the project team shall check whether the study is carried out in accordance with the test plan. The informed consent and screening inclusion of subjects should be checked regularly during the test. Ensure that all data records are filled in correctly and consistent with the original data; All errors or omissions have been corrected or noted, signed and dated by the investigator; The subject's special situation during the study should be confirmed and recorded; Confirm that all adverse events should be recorded, and serious adverse events should be reported, recorded and handled in a timely manner within the specified time. 3.
