药物成份或治疗方案详述: |
髓芯减压术联合3D打印钛笼(AVN CAGE)及人工骨治疗早期股骨头缺血性坏死的主要材料及手术步骤如下,AVN CAGE是3D打印的中空结构,基体材料为符合临床应用的Ti6Al4V钛合金,主要用于支撑、填充、修复患者病变的股骨头,已获批进入临床使用。人工骨选用Wright公司的PRO-DENSE或百赛公司的 GeneX人工骨。手术治疗流程上,髋部常规消毒铺巾,透视引导下确定股骨大粗隆下减压骨通道入点及减压通道方向。使用专用股骨头坏死钻孔减压手术器械,制备减压骨通道,去除坏死骨,冲洗后,植入AVN CAGE,再于钛笼内部及骨通道加压注入人工骨,待人工骨硬化后,冲洗缝合伤口,结束手术。 |

Description for medicine or protocol of treatment in detail: |
The material and surgical process are describe as following. The AVN CAGE was a 3D-printed porous titanium cage with a hollow bullet-like structure and was made of Ti6Al4V. Medical registration certification of AVN CAGE has been issued (AK Medical Co., Ltd ,Beijing, China) . Two types of artificial bone could been used in this study, including PRO-DENSE? artificial bone (Wright Medical Technology?, Arlington, TN, USA) and Genex Bone Graft Substitute (Biocomposites, Keele, England). After the surgical filed has been disinfected, a standardized CD procedure was performed with consistently located target points and entrances. The target points were located at the necrotic areas of the femoral head, which was commonly the anterosuperolateral portion. The entrance was the predrilled hole located at a consistent position on the lateral wall of the great trochanter. A 3-mm guidewire was inserted from the entrance to the target point through a specifically made guiding jig. Two cannulated reamers (7.5 mm and 9.5 mm in diameter, respectively) were then used to remove bone up to the subchondral level following the guidewire. The debris of bone foam was cleaned with normal saline lavage. Bone graft was injected into the chamber of AVN CAGE and bone channel. After the bone graft has become solid, incision were washed and sutured. |
