本研究以膝关节半月板损伤患者人群为研究对象,拟设计的一组以骨髓释放技术(BMVP)为干预措施的半月板损伤患者单纯半月板缝合与联合BMVP的随机对照实验并行合并BMVP与合并前交叉韧带重建(ACL-R)后对半月板愈合影响的前瞻性队列研究评估骨髓内容物对半月板愈合的影响,对比骨髓内容物刺激作用与联合韧带重建后半月板高愈合率的关系,旨在探究骨髓释放技术释放的骨髓内容物的刺激是否对半月板损伤的愈合起促进作用,骨髓释放技术释放的骨髓内容物的刺激对半月板愈合的作用是否与韧带重建时对半月板愈合的促进作用相似。 |

Objectives of Study: |
This study, patients with meniscus injury were selected as the research objects and the bone marrow venting procedure(BMVP) as the intervention, a randomized controlled trial combined prospective cohort study was designed to compare the effects of meniscal repairing combined with BMVP and meniscus repairing alone on meniscus healing in patients with meniscus injury to evaluate the effect of bone marrow content on meniscus healing and compare the relationship between bone marrow content stimulation and high meniscus healing rate after combined ligament reconstruction The purpose of this study was to investigate whether the stimulation of bone marrow contents released by bone marrow release technology could promote meniscus healing, and whether the stimulation of bone marrow contents released by bone marrow release technology could promote meniscus healing similar to that of ligament reconstruction. |

研究设计 以膝关节半月板损伤患者人群为研究对象,以髁间窝骨髓释放术(Bone Marrow Venting Procedure, BMVP)为干预措施 随机化与盲法 使用Microsoft Excel生成随机数表,使用REDCap医学研究电子数据采集平台随机化模块上传随机分组CSV表,在纳入研究对象时自动分配干预方式,干预方式对研究对象隐藏。 试验分组 对照组A:行单纯半月板缝合的半月板损伤患者 对照组B:行半月板缝合+交叉韧带重建的半月板损伤合并韧带损伤患者 实验组E:行单纯半月板缝合+髁间窝骨髓释放的半月板损伤患者 干预措施 骨髓释放技术(Bone Marrow Venting Procedure, BMVP) 操作流程: ①膝关节镜下正常缝合修复损伤的半月板 ②半月板修复完成后,将软骨修复微骨折骨锥或2.0克氏针经关节镜微创操作通道置入关节腔 ③在髁间窝前交叉韧带和后交叉韧带足印区前缘各制备三处间隔约5mm,深度约5mm的骨髓释放骨道,至关节镜下中断冲洗液时可见骨髓内容物从骨道溢出 ④术毕常规缝合,术后按照康复计划康复 临床数据收集 基于REDCap医学研究电子数据采集平台(REDCap 12.3.0 - 2022 Vanderbilt University)设计患者评分与术中半月板损伤信息收集表单,包括Tegner膝关节运动评分、Lysholm膝关节功能评分、VAS疼痛评分、2000国际膝部文件委员会(IKDC)主观膝部评估表;术中根据国际关节镜、膝关节外科和骨科运动医学学会(ISAKOS)半月板撕裂分类收集患者半月板损伤信息。 随访流程 术后,每组受试者均通过网络或电话的形式进行随访; 术后每3个月随访受试者膝关节功能评分(至术后5年); 术后第6个月建议受试者复查患者膝关节MRI,留取图像。 |

Description for medicine or protocol of treatment in detail: |
Study design Bone Marrow Venting Procedure (BMVP) was used as the intervention in patients with knee meniscus injury Randomization and blindness Microsoft Excel was used to generate the random number table, and the randomization module of REDCap Medical Research electronic data acquisition platform was used to upload the randomly grouped CSV table. Intervention methods were automatically assigned when the subjects were included, and the intervention methods were hidden from the subjects. Group Control group A: meniscus injury patients treated with isolated meniscus repairing Control group B: patients with meniscus injury combined ligament injury treated with meniscus repairing & cruciate ligament reconstruction Experimental group E: meniscus injury patients treated with meniscus repairing & BMVP Interventions Bone Marrow Venting Procedure (BMVP) Operation process: (1)Repairing the injured meniscus with under knee arthroscopy (2)After meniscus repairing, bone awl or 2.0 K-wire was inserted into the articular cavity through arthroscopic operation channel (3)Three bone marrow releasing channels with an interval of about 5mm and a depth of about 5mm were prepared on the edge of the footprint area of anterior cruciate ligament and posterior cruciate ligament in intercondylar fossa, till the bone marrow contents spilled from the bone canal when the irrigation fluid was interrupted under arthroscopy (4)Routine sutures were performed after the operation, and the patients recovered according to the normal postoperative rehabilitation plan Clinical data collection Patient score and intraoperative meniscus injury information collection form were designed based on REDCap Medical Research Electronic Data Acquisition platform (REDCap 12.3.0-2022 Vanderbilt University). Including Tegner score, Lysholm score, VAS pain score, 2000 International Knee Document Committee (IKDC) subjective knee assessment Form; Intraoperative meniscal injury information was collected according to the International Society for Arthroscopy, Knee Surgery and Orthopaedic Sports Medicine (ISAKOS) Meniscal tear classification. Follow-up process After surgery, all subjects in each group were followed up via Internet or telephone. Knee function scores were followed up every 3 months after surgery (up to 5 years after surgery). At the 6th month after surgery, the subjects were advised to review the MRI of the injured knee. |
