研究目的：

建立"互联网+"妊娠期糖代谢紊乱高危孕母筛查路径，建立"互联网+"妊娠期疾病管理模式，通过多中心实效性阶梯式整群随机对照试验验证这种诊疗模式对改善不良妊娠结局的作用。  

Objectives of Study：

To establish the "Internet +" clinical management pathway for high-risk pregnant women with glucose metabolic disorder, and to establish the "Internet +" pregnancy disease management model, and to verify the role of this management model in improving adverse pregnancy outcomes through multi-center stepped-wedge cluster-randomized trial.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

处于孕早期（＜14周）且满足以下任何一点者可视为合格的入选病人：
1. BMI≥23kg/m2；
2. 既往有GDM病史或巨大儿（新生儿出生体重≥4000g）生产史；
3. 既往有糖耐量异常；
4. 既往有多胎妊娠；
5. 2型糖尿病家族史（特别是1级亲属）；
6. PCOS（多囊卵巢综合征）；
7. 本次/计划使用辅助生殖技术受孕；
8. 近3月口服/静脉使用糖皮质激素；
9. 孕前有高脂血症、高血压。

Inclusion criteria

Patients who are in early pregnancy (< 14 weeks) and meet any of the following criterion can be enrolled in this study:
1. BMI >= 23kg/m2;
2. Previous history of GDM or birth history of macrosomia (birth weight >= 4000g);
3. Previous abnormal glucose tolerance;
4. Previous multiple pregnancies;
5. Family history of type 2 diabetes (especially the direct relatives);
6. PCOS (polycystic ovary syndrome);
7. This time / plan to use assisted reproductive technology for pregnancy;
8. Oral / intravenous glucocorticoids in recent 3 months;
9. With hyperlipidemia and hypertension before pregnancy.

排除标准：

1. 孕前糖尿病（包括孕早期OGTT符合糖尿病诊断标准）；
2. 研究者认为存在可能妨碍数据有效性和安全性的受试者；
3. 不能配合完成全孕程和产后随访；
4. 危重孕产妇分级达3-4级孕妇。

Exclusion criteria：

1. Diagnosis with diabetes before pregnancy;
2. Subjects who may have ineffective and unsafe data;
3. Could not complete the whole pregnancy process and follow-up;
4. Critically ill pregnant women who are classified as grade 3-4 degree.

按照实效性阶梯式分层整群随机原则，在安徽省糖尿病联盟65家医院中随机挑选9家医院，根据地理区域结合诊疗级别分为10个集群。再通过REDcap的随机模块将干预随机分到各个集群。

According to the effectiveness of stepped-wedge cluster-randomized trial, 9 hospitals were randomly selected in 65 hospitals of Anhui diabetes alliance. According to the geographical area combined with the diagnosis and treatment level, interventions were randomly assigned to clusters through REDcap's randomization module.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

根据原始数据的观察记录，将数据及时、完整、正确、清晰地记录病例报告表中。EDC录入数据时采用双录入法比对核查。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

According to the observation records of the original data, the data shall be recorded in the case report form in a timely, complete, correct and clear manner. EDC adopts double entry method for comparison and verification when entering data. Electronic data files shall be stored by category, and multiple backups shall be stored on different disks or recording media, which shall be properly stored to prevent damage.
