研究目的：

本研究目的为评估右美托咪啶复合布比卡因行骶管阻滞麻醉对婴幼儿单侧腹股沟斜疝修补术术中疝囊牵拉反射的影响  

Objectives of Study：

The study was done to evaluate the effect of caudal bupivacaine supplemented with caudal dexmedetomidine on the response of hernial sac traction in children undergoing unilateral inguinal hernia repair.

药物成份或治疗方案详述：

右美托咪啶1μg/kg，0.25%布比卡因1ml/kg 

Description for medicine or protocol of treatment in detail：

dexmedetomidine 1 μg/kg, 0.25%bupivacaine 1ml/kg 

纳入标准：

60例择期行单侧腹股沟斜疝疝修补术的患儿，ASA Ⅰ级，年龄1-6岁，以双盲方式随机分为2组。

Inclusion criteria

Sixty children (ASA status I patients, aged 12 to 72 months) scheduled for elective surgery for unilateral inguinal hernia repair, were evenly and randomly assigned into two groups in a double-blinded manner.

排除标准：

发育迟缓病史、出血体质、既往数周有阿司匹林服药史、对布比卡因或右美托咪啶过敏、先天性后背部异常、骶管阻滞穿刺区域感染、存在系统性疾病、及患儿感冒等

Exclusion criteria：

Exclusion criteria consisted of a history of developmental delay; bleeding diathesis; aspirin ingestion within the previous week; a known allergy to any of the study drugs; congenital anomaly of lower back; any signs of local infection in the caudal region; systemic disease; and common cold.

盲法：

麻醉医生B负责给患儿实施麻醉及骶管阻滞，所用骶管阻滞药物由麻醉医生A配制，麻醉医生B不知道所用骶管阻滞药物成分；术中患儿心率、收缩压监测由麻醉医生B实施；术中疝囊牵拉时需用氯胺酮干预的患儿由麻醉医生B记录；患儿进入PACU后由一名资深护士C使用CHIPPS评分对患儿进行术后疼痛评分。麻醉医生B和护士C不知道病人分组情况。

Blinding：

The administement of the anesthesia,and the recored for the change of HR,SAP during operation were performed by anesthesiologist B. The drug used for caudal block was prepared by anesthesi-ologist A. In PACU,Postoperative pain was asse-ssed by an experienced nurse who was unaware of the study solutions using Children’s and Infant’s Postoperative Pain Scale (CHIPPS).Anesthesio-logist B and nures C donot know the patients attribute to which group.
