主要目的：采用统一标准和规范，构建整合临床表型、疾病诊疗信息、随访信息、临床样本及其生命组学信息等多层次数据的大脏器移植数据管理、共享平台和生物样本全息库，汇聚、整编和融合大脏器移植领域各层面数据资源，最终实现在大脏器移植领域面向临床诊疗、精准医学研究和生物医药产业的信息集成共享服务，满足临床诊疗服务与临床科学研究“双轨发展”的需要。  

Objectives of Study：

Main purpose: Adopt unified standards and norms to build a large organ transplantation data management, sharing platform and biological sample holographic database integrating multi-level data such as clinical phenotype, disease diagnosis and treatment information, follow-up information, clinical samples and their life omics information, so as to gather, integrate and integrate data resources at all levels in the field of large organ transplantation, and finally realize clinical diagnosis and treatment in the field of large organ transplantation The information integration and sharing services of precision medical research and biomedical industry meet the needs of "dual track development" of clinical diagnosis and treatment services and clinical scientific research.

纳入标准：

2021.12-2022.9在项目单位因各种终末期肝肾心肺行移植的成人和儿童，包括：尸体移植、活体移植，且患者术后依从性良好、按医嘱服用抗排斥药物及其它合并用药。

Inclusion criteria

Adults and children undergoing various end-stage liver, kidney, heart and lung transplantation in the project unit from December 2021 to September 2022, including cadaver transplantation and living donor transplantation, with good postoperative compliance, taking anti rejection drugs and other combined drugs according to the doctor's instructions.

排除标准：

1.围手术期死亡或移植术后预计生存时间小于3个月；
2.患者或家属拒绝签署知情同意书。

Exclusion criteria：

1.Perioperative death or estimated survival time after transplantation is less than 3 months;
2.Patients or family members refused to sign informed consent.
