研究目的：

探索总结脑卒中、颅脑外伤、脑肿瘤所致局灶性脑损伤相关功能障碍的康复预后规律；为指导判断临床康复预后、制定康复评定和治疗方案提供依据。  

Objectives of Study：

To explore the prognosis factors of functional impairments due to stroke, brain injury or brain tumor, in order to instruct the clinical rehabilitation assessments and treatments.

纳入标准：

（1）脑卒中
入选标准：①患者年龄18-75岁；②脑梗死或脑出血；③诊断符合1995年全国第四届脑血管病学术会议通过的各类脑血管病诊断要点；④经头颅CT或MRI检查证实脑卒中；⑤经客观功能评定提示运动、语言/言语、认知、吞咽功能障碍、神经源性膀胱/肠、精神行为异常中的一项或几项；⑥愿意配合康复评定、治疗和随访；⑦患者本人签署或由其法定代理人代签知情同意书。
（2）颅脑外伤
入选标准：①患者年龄18-75岁；②明确颅脑外伤病史；③经头颅CT或MRI检查证实存在脑挫裂伤、（创伤性）脑实质内出血、弥漫性轴索损伤等累及脑实质的损伤；④经客观功能评定提示运动、语言/言语、认知、吞咽功能障碍、神经源性膀胱/肠、精神行为异常中的一项或几项；⑤愿意配合康复评定、治疗和随访；⑥患者本人签署或由其直系亲属代签知情同意书。
（3）脑肿瘤
入选标准：①患者年龄18-75岁；②经可靠的影像学检查或病理证实脑肿瘤诊断；③经头颅CT或MRI检查证实脑肿瘤累及脑实质（占位、侵犯、手术范围累及脑实质等）；④经客观功能评定提示运动、语言/言语、认知、吞咽功能障碍、神经源性膀胱/肠、精神行为异常中的一项或几项；⑤愿意配合康复评定、治疗和随；⑥患者本人签署或由其直系亲属代签知情同意书。

Inclusion criteria

(1)Stroke
Inclusion criteria: ① age 18-75 years; ②diagnosed as ischemic or hemorrhagic stroke; ③ the diagnosis of stroke was according to the 1995 criteria from the 4th Academic Conference on cerebrovascular diseases; ④ stroke verified by head CT or MRI; ⑤ verified to suffer from one or more than one types of functional impairments, including motor, language/speech, cognitive impairments, dysphagia, neurological bladder/bowel, and psychiatric or behavioral symptoms; ⑥ be able to cooperate with rehabilitation assessments, treatments and follow-up; ⑦ written informed consent obtained from the participant or his/her legal representative(s).
(2)Brain injury
Inclusion criteria: ① age 18-75 years; ② brain injury history; ③ the diagnosis of brain injury verified by head CT or MRI; ④ verified to suffer from one or more than one types of functional impairments, including motor, language/speech, cognitive impairments, dysphagia, neurological bladder/bowel, and psychiatric or behavioral symptoms; ⑤ be able to cooperate with rehabilitation assessments, treatments and follow-up; ⑥ written informed consent obtained from the participant or his/her legal representative(s).
(3)Brain tumor
Inclusion criteria: ① age 18-75 years; ②diagnosed as brain tumor, verified by imaging and/or pathological examination; ③ lesions involving brain tissues, verified by CT or MRI; ④ verified to suffer from one or more than one types of functional impairments, including motor, language/speech, cognitive impairments, dysphagia, neurological bladder/bowel, and psychiatric or behavioral symptoms; ⑤ be able to cooperate with rehabilitation assessments, treatments and follow-up; ⑥ written informed consent obtained from the participant or his/her legal representative(s).

排除标准：

①意识不清不能配合检查及治疗的患者；②生命体征不平稳不适合进行康复评定、治疗者；③合并控制不良的非器质性精神障碍，如精神分裂症、双相情感障碍；④存在重要脏器功能衰竭或病情危重等对患者生存质量的影响超过脑损伤所致功能障碍的疾病或状态；⑤存在神经系统感染、自身免疫性疾病、变性病、遗传代谢病等导致神经精神障碍的疾病；⑥不能完成基本疗程，依从性可能不好者（即不能坚持治疗）及难以随访者。

Exclusion criteria：

①not able to cooperate with the assessment and/or treatment due to unconscious②unstable vital signs that be not able to accept assessment and/or treatment③suffered from uncontrolled non organic psychosis, such as schizophrenia or bipolar disorder; ④suffered from vital organ failure which influence the patient’s health condition more than the brain lesions; ⑤comorbid other severe neurological disorders, including neurological inflammation, neurological autoimmune disease, neurodegenerative disease, inherited metabolic disease; ⑥ not able to complete the basic course of treatment and/or follow-up.
