研究目的：

研究上肢负重联合局部肌肉振动仪（Segmental Muscle Vibration，SMV）对脑卒中伴肩关节半脱位患者缓解疼痛、改善肩周肌肉张力、增强肩周肌肉力量、减轻脱位程度、增强肩关节本体感觉及提高上肢功能的疗效，为脑卒中后肩关节半脱位的康复治疗提供新的方案。  

Objectives of Study：

In this study, upper extremity weight-bearing combined with segmental muscle vibration (SMV) was used to observe the effect on relieving pain, improving muscle tension around the shoulder, enhancing muscle strength around the shoulder, reducing the degree of dislocation, enhancing shoulder proprioception and improving upper limb function in patients after stroke with shoulder subluxation, so as to provide a new program for rehabilitation of shoulder subluxation after stroke.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合中华医学会通过的《中国急性缺血性脑卒中诊治指南2018》中脑卒中诊断标准，且经头颅CT或MRI检查证实，生命体征稳定；
（2）肩关节半脱位诊断标准符合《中国康复医学诊疗规范》中关于肩关节半脱位的诊断，即临床触诊示肩峰下沉或触及凹陷，肩胛下角降低，呈翼状肩，上肢下垂时肩部不适或疼痛感明显，肩部被动托起时不适感或疼痛感减轻，且经Ｘ线片或肌骨超声证实；
（3）首次发病，病程＜12个月；
（4）单侧发病，患侧上肢Brunnst-rom分期为Ⅰ-Ⅱ期；
（5）年龄20-80岁；
（6）患者意识清楚，无认知功能障碍，且配合检查和治疗；
（7）所有受试者均签署知情同意书。

Inclusion criteria

1. Meet the diagnostic criteria for stroke in the "Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke 2018" adopted by the Chinese Medical Association, and confirm that the vital signs are stable by cranial CT or MRI examination;
2. The diagnostic criteria for shoulder subluxation are in line with the diagnosis of shoulder subluxation in the "Chinese Rehabilitation Medicine Diagnosis and Treatment Code", that is clinical palpation shows that the shoulder peak is sunken or touched, the lower angle of the shoulder blade is reduced, the winged shoulder is formed, the shoulder discomfort or pain is obvious when the upper limb droops, the discomfort or pain is reduced when the shoulder is passively lifted, and confirmed by X-ray or musculoskeletal ultrasound;
3. First onset, the course of the disease < 12 months;
4. Unilateral onset, the upper limb Brunnst-rom stage of the affected side is stage I-II.;
5. The patient is ranged from 20 to 80;
6. The patient is conscious, without cognitive dysfunction, and cooperates with examinatian and treatment;
7. All participants sign informed consent.

排除标准：

（1）患者依从性差，或严重认知功能障碍不能配合治疗者；
（2）心、肺、肾等重要脏器严重功能不全；
（3）既往存在肩部功能障碍病史，如发病前存在肩陈旧性脱位、骨折等病史；
（4）其他疾病原因导致上肢肩关节半脱位。

Exclusion criteria：

1. Patients with poor compliance, or severe cognitive dysfunction can not cooperate with the treatment;
2. Patients with serious dysfunction of heart, lung, kidney and other important organs;
3. Patients with previous history of shoulder dysfunction, such as the presence of shoulder obsolete dislocation, fractures and other medical history before the onset of the disease;
4. Other disease or causes lead to upper limb shoulder joint subluxation.
