研究目的：

1.观察卒中后失语症患者语言损伤特征，探索其命名功能障碍的内在机制； 2.探讨头皮针对失语症患者命名功能及腹-背侧脑网络的影响。  

Objectives of Study：

1. To observe the characteristics of language impairment in aphasia patients after stroke and explore the internal mechanism of naming dysfunction; 2. To investigate the influence of scalp on the naming function and ventral-dorsal brain network in aphasia patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①脑卒中患者经颅脑CT或MRI检查证实为左脑梗死;；
②首次脑卒中，6个月≤病程≤2年；
③18岁≤年龄≤80岁；
④由左侧大脑半球脑梗死所导致，经波士顿诊断性失语症严重程度分级标准评定为1-5级的患者，且由波士顿命名测试判定为命名功能困难者；
⑤右利手；
⑥志愿签署知情同意书，能理解、配合本项试验的康复评定及治疗。

Inclusion criteria

① Stroke patients have cerebral infarction in the left cerebral hemisphere confirmed by craniocerebral CT or MRI examination;
② The patient had a stroke for the first time, and disease duration was within 6 months to 2 years;
③ Patients aged 18~80 years;
④ Patients rated by the Boston Diagnostic Aphasia Examination (BDAE) Scale as Grade 1-5, and identified by the Boston Naming Test(BNT) as having naming impairment;
⑤ Right-handed people;
⑥ Participants agree to sign the informed consent form, and could understand and cooperate with the rehabilitation assessment and treatment in this study.

排除标准：

①由其它疾病引起的失语者（比如脑肿瘤、脑外伤、脑寄生虫病等）；
②由构音障碍等周围器官病变所致言语障碍患者；
③有严重认知障碍无法配合语言功能检查患者；
④存在严重的听觉或视觉功能障碍的失语患者；
⑤存在卒中后严重并发症（比如严重肺部感染、肩手综合征、下肢静脉栓塞等患者）；
⑥患有严重心脏疾病，心、肝、肾功能衰竭，恶性肿瘤、消化道出血的患者；
⑦癫痫症病史者。

Exclusion criteria：

① Aphasia is caused by other diseases (such as brain tumor, brain trauma, brain parasitic disease, etc.);
② Language dysfunction caused by dysarthria and other peripheral organ diseases;
③ Patients with severe cognitive impairment who cannot cooperate with language function assessment;
④ Aphasia patients with severe auditory or visual dysfunction;
⑤ Patients have serious complications after stroke (such as severe pulmonary infection, shoulder-hand syndrome, deep venous thrombosis, etc.);
⑥ Patients with serious heart disease, organ failure （heart, liver or kidney）, malignant tumor or gastrointestinal bleeding;
⑦ Patients have a history of epilepsy;

常规内科治疗，具体包括病因的处理、血压、血糖、血脂的管理、抗血小板、抗凝治疗等；康复治疗，包括常规物理治疗和作业治疗，以及听力、理解、复述、命名、阅读等言语语言治疗；健康教育，向受试者讲授卒中后失语症的基本概念、患病后生活注意事项以及针对性的康复治疗措施等。

基础治疗同对照组。头皮针取穴为：顶颞前斜线下2/5和颞前线。使用华佗牌直径0.30 mm，长25 mm无菌针灸针（苏州医疗用品厂有限公司），刺入3cm左右为宜，留针30分钟。

本次研究受试者随机分组序列由本校循证医学中心统计工作人员采用SAS9.1 统计软件PROC PLAN 程序产生，并应用不透光、密闭封信法隐藏随机分组序列。按照各组1:1 的比例随机分组，将阈下抑郁受试者分入太极拳组、等待名单组。预先设定产生的随机数字范围为A、B 两组，根据随机数字范围将受试者分配至A 组或B 组，运行SAS 统计软件的PROC PLAN 过程生成随机数字表，将程序产生的随机分组结果打印出来，制订编码。参与随机分配序列产生与保管的人员不参与招募活动，由课题管理者通过电话告知合格参与者分配结果，随机序列由课题组专人负责管理。

The randomized sequence of the subjects in this study was generated by the statistical staff of the School of Evidence-Based Medicine using the SAS9.1 statistical software PROC PLAN program, and the opaque and closed letter method was used to hide the random grouping sequence.
