研究目的：

现有的研究中并没有指出最为有效的肺萎陷技术和断开时间。本研究拟对三种有效性较高的方法进行对比，评价三种方式的有效性和并发症发生率，从而获得最佳的肺萎陷方式及操作方案，包括有效性、外科医生满意度和患者并发症等，为今后临床工作带来更加精细化的麻醉管理策略。  

Objectives of Study：

Existing studies have not identified the most effective lung collapse technique and disconnection time. This study aims to compare the three effective methods and evaluate the effectiveness and complication rate of the three methods, so as to obtain the best pulmonary collapse method and operation plan, including effectiveness, surgeon satisfaction and patient complications. In order to bring more refined anesthesia management strategies for future clinical work.

纳入标准：

年龄大于18周岁，小于65周岁
ASA分级I-Ⅲ
择期手术患者
接受胸部手术时使用LDLT
需要单肺通气
签署知情同意书

Inclusion criteria

Aged between 18 and 65
ASA grade I - Ⅲ
Patients undergoing elective surgery
LDLT is used for chest surgery
One lung ventilation is required
Informed consent

排除标准：

预期插管困难(Mallampatti评分≥3)
FEV1<预测值的70%
慢性阻塞性肺疾病(COPD)
气胸
肺大泡
胸膜粘连
严重哮喘
胸部手术史
纽约心脏协会(NYHA)心力衰竭III~IV级
脑血管疾病

Exclusion criteria：

Expected difficulty with intubation (Mallampatti score ≥3)
FEV1< 70% of the predicted value
Chronic obstructive pulmonary Disease (COPD)
pneumothorax
Pulmonary bullae
Pleural adhesions
Severe asthma
History of chest surgery
New York Heart Association (NYHA) class III to IV heart failure
Cerebrovascular disease

DLT通向非通气肺的管腔在患者转侧卧位后立即夹闭，开始单肺通气，而DLT管腔的远端直到胸膜开放后才向大气开放。

The lumen leading to the non-ventilated lung of the DLT is clamped immediately after the patient is transferred to lateral decubitus to initiate single-lung ventilation, while the distal end of the DLT lumen is not open to the atmosphere until the pleura is opened.

The patient was changed to a lateral decubitus position, and pure oxygen was used for double-lung ventilation until the pleura was incised. One-lung ventilation was started 1 min after the DLT was disconnected at the time of pleural incision.
