研究目的：

采用随机对照试验的临床研究方法，证实口服尼可地尔对急性心肌梗死急诊PCI患者冠脉微循环障碍具有治疗作用，为尼可地尔治疗冠脉微循环障碍提供国人循证医学证据。  

Objectives of Study：

Using the clinical research method of randomized controlled trial, it is confirmed that oral nicorandil has a therapeutic effect on coronary microcirculation disorder in patients with acute myocardial infarction undergoing emergency PCI, so as to provide evidence-based medical evidence for nicorandil in the treatment of coronary microcirculation disorder.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.12小时内的新发胸痛（18小时内的以心源性休克为表现的急性心肌梗死），胸痛持续超过30分钟；
2.心电图表现：①至少两个相邻导联 J 点后新出现 ST 段弓背向上抬高 [V2-V3 导联≥ 0.25 mV（＜ 40 岁男性）、≥ 0.2 mV（≥ 40 岁男性）或≥ 0.15 mV（女性），其他相邻胸导或肢体导联≥ 0.1 mV]伴或不伴病理性 Q 波、R 波减低；②新出现的完全左束支传导阻滞；③超急性期 T 波改变；
3.伴或不伴有肌钙蛋白I升高。

Inclusion criteria

1. New onset chest pain within 12 hours (acute myocardial infarction with cardiogenic shock within 18 hours), chest pain lasting for more than 30 minutes;
2. ECG findings: ① ST segment arcuate back elevation newly appeared after J point of at least two adjacent leads [v2-v3 leads ≥ 0.25 MV (< 40 year old male), ≥ 0.2 MV (≥ 40 year old male) or ≥ 0.15 MV (female), other adjacent chest leads or limb leads ≥ 0.1 MV] with or without pathological Q wave and R wave reduction;② New complete left bundle branch block; ③ T wave changes in hyperacute phase;
3. With or without elevated troponin I.

排除标准：

1.血管狭窄严重无法行PCI治疗或需接受急诊搭桥者；
2.院前接受溶栓治疗者；
3.入院24小时内死亡者。

Exclusion criteria：

1. Patients with severe vascular stenosis who cannot undergo PCI or need emergency bypass;
2. Those who received thrombolytic therapy before hospital;
3. Those who died within 24 hours after admission.
