研究目的:

神经外科开颅术患者术前焦虑紧张可激活交感神经系统导致血流动力学剧烈波动,增加术后并发症。右美托咪定是一种高选择性α2肾上腺素受体激动剂,具有可唤醒镇静、镇痛、抗焦虑、抗交感、神经保护作用且几乎无呼吸抑制的特点,使其成为神经外科极具优势的麻醉辅助剂。本项目采用静息态功能磁共振成像技术(rsf-MRI)研究右美托咪定在良性脑肿瘤患者脑内引起的神经网络变化,明确其调控活体神经网络的机制,为右美托咪定的抗焦虑及其脑保护机制研究提供神经网络方面的证据。  

Objectives of Study:

Preoperative stress and anxiety in patients with brain tumors can activate the sympathetic nervous system, leading to violent fluctuations in hemodynamics and increasing postoperative complications.Dexmetomidine is a highly selective alpha-2 adrenergic receptor agonist with a large number of advantages,including arousal sedation, analgesia, anxiolytic, antisympathetic, neuroprotective effects and little respiratory inhibition,making it an advantageous anesthetic adjuvant for neurosurgery. This project, using resting-state functional magnetic resonance imaging (rsf-MRI),is going to study the brain neural network changes caused by dexmedetomidine in patients with benign brain tumors,and to clarify its mechanism of regulating the neural network in vivo,providing evidences in brain neural network for studys of anti-anxiety and brain protection mechanism of dexmetomidine.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1. 年龄18~64岁;
2. 良性脑肿瘤患者,瘤体局限不压迫主要脑功能网络区(如DMN、ECN及FPN等),且拟行择期手术治疗;
3. ASA分级I~II级;
4. BMI 18~30kg/㎡;
5. 右利手。

Inclusion criteria

1. Aged 1864 years old;
2. Patients with benign brain tumors, which is limited and does not compress the main brain functional network areas (such as DMN, ECN, FPN, etc.),and will performming surgery;
3. ASA (American Society of Anesthesiologists) physical status I or II grade;
4. BMI 1830kg/m^2;
5. Right-hand.

排除标准:

1. 近期有全身麻醉史;
2. 有核磁扫描禁忌;
3. 存在颅内病变或全身合并症;
4. 有麻醉禁忌症及药物过敏史;
5. 有神经或精神疾病史或者有大脑结构异常的病史;
6. 临床评估为恶性脑肿瘤;
7. 孕产妇或哺乳期女性;
8. 有吸毒或酗酒史;
9. 右美泵注期间HR或BP下降超过20%的(此时紧急干预)患者;
10.拒绝参加本研究者。

Exclusion criteria:

1. Recent history of general anesthesia;
2. Contraindications to MRI;
3. Intracranial lesions or systemic comorbidities;
4. History of anesthetic contraindications and allergyic reactions;
5. History of neurological or psychiatric disorders or structural abnormalities of the brain;
6. Evaluated as malignant brain tumor;
7. Pregnant or lactating women;
8. History of drug or alcohol abuse;
9. Patients with HR or BP decreased by more than 20% during dexmedetomidine infusion (emergency intervention at this time);
10.Refusing to participate in this researcher.
