研究目的:

1. 通过超声多模态成像技术评估先心病胎儿心脏构型改变及早期心功能损伤,将心肌力学指标联合多种超声参数构建胎儿产前诊断及预后风险模型,为产前产后一体化管理提供依据。 2. 结合弥散张量成像技术和分子病理学评估先心病胎儿心肌纤维排列及心肌微结构变化,探讨先心病胎儿心肌力学改变与病理变化的内在联系,为胎心应变成像评价心功能建立病理金标准。 3. 探究全基因组测序在先天性心脏病胎儿中的产前诊断量;鉴定新的先心病致病基因;构建先心病胎儿心脏细胞发育图谱。  

Objectives of Study:

1. To construct prenatal diagnostic and prognostic risk models by evaluation of fetal myocardial mechanics in congenital cardiac malformations, which combine with conventional fetal echocardiography parameters. 2. To explore the intrinsic mechanism and establish a pathological gold standard between myocardial mechanical dysfunction and pathological changes in congenital heart disease, diffusion tensor imaging and molecular pathology would be performed to assess myocardial fiber arrangement and myocardial microstructural changes. 3. To construct an atlas of fetal heart cell development in congenital heart disease, we would explore the prenatal diagnostic yield of whole-genome sequencing and identify novel causative genes for congenital heart disease.

纳入标准:

引产胎儿标本:
1. 签名及日期的知情同意书;
2. 因胎儿先心病或者其他原因自愿在协和医院引产并同意将引产后死胎捐赠给本课题组。

各类先心病胎儿:
1. 签名及日期的知情同意书;
2. 承诺遵守研究程序,并配合实施全过程研究;
3. 产前超声心动图诊断或疑诊为某种类型先心病。

正常对照组胎儿:
1. 签名及日期的知情同意书;
2. 孕周与各类先心病胎儿相匹配;
3. 胎儿超声心动图检查心内结构正常。

Inclusion criteria

The specimens of fetuses:
1. Signed informed consent;
2. Donate the fetus specimens voluntarily after the termination of pregnancies in Union Hospital due to fetal congenital heart diseases or other reasons.

All subtypes of fetuses with congenital heart disease:
1. Signed informed consent;
2. Commit to abide by the research procedures and cooperate with the implementation of the whole process of the research;
3. Diagnosed or suspected a certain type of congenital heart disease by prenatal echocardiography.

Fetuses in the normal control group:
1. Signed informed consent;
2. The gestational age should match the fetuses with congenital heart diseases;
3. Heart structure is normal and checked by echocardiography after birth.

排除标准:

引产胎儿标本:
1. 捐献者或法定代理人要求退出临床研究;
2. 引产相关事宜及引产后死胎捐赠流程及手续不符合相关法律法规。

各类先心病胎儿:
1. 双胎或多胎妊娠;
2. 胎儿宫内生长受限;
3. 胎儿持续性心律失常;
4. 除患某类所研究先心病外合并其余严重心内及心外畸形;
5. 母体基础疾病及妊娠合并疾病(糖尿病、高血压、甲状腺功能异常等)。

正常对照组胎儿:
1. 双胎或多胎妊娠;
2. 心律异常;
3. 心外畸形;
4. 基因、染色体异常;
5. 母体基础疾病及妊娠合并疾病(糖尿病、高血压、甲状腺功能异常等)。

Exclusion criteria:

The specimens of fetuses:
1. Donor or legal representative requests to withdraw from clinical research;
2. The donation procedures after induction of labor do not comply with relevant laws and regulations.

All fetuses with congenital heart disease:
1. Twin or multiple pregnancies;
2. Intrauterine growth restriction of the fetus;
3. Persistent fetal arrhythmia;
4. Except for a certain type of congenital heart disease with other intracardiac and extracardiac malformations;
5. Maternal diseases and complications of pregnancy (diabetes, hypertension, abnormal thyroid function, etc.).

The fetuses in the normal control group:
1. Twin or multiple pregnancies;
2. Abnormal heart rhythm;
3. Extracardiac deformations;
4. Gene and chromosome abnormalities;
5. Maternal diseases and complications of pregnancy (diabetes, hypertension, abnormal thyroid function, etc.).
