研究目的：

（1）调查社区常见老年慢性病患者居家用药安全知信行的现况； （2）构建出适合社区老年慢性病患者的用药安全健康教育内容； （3）比较常规健康教育、回授法健康教育、家庭参与式健康教育及家庭参与式的回授法健康教育4种教育模式对社区老年慢性病患者用药安全的作用效果。  

Objectives of Study：

(1) Investigate the current status of knowledge, belief, and practice of home medication safety among elderly patients with chronic diseases in the community; (2) Constructing medication safety and health education content suitable for elderly patients with chronic diseases in the community; (3) To compare the effect of four educational modes: conventional health education, teaching-back health education, family-participating health education, and family-participating teaching-back health education on drug safety of elderly patients with chronic diseases in the community.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者：①年龄≥60周岁；②在福州市城市社区居住时间≥6个月；③有与家属共同居住者；④已被二级及以上医院确诊为高血压或糖尿病。
家庭照顾者：①年龄≥18岁；②愿意参加本研究，能进行正常沟通交流；③与老年慢性病患者共同居住，并承担其主要的日常生活照顾工作的家属；④若患者同时有几个家庭照顾者照顾，则选择照顾时间最长的一位家属。

Inclusion criteria

Patients: ①Age ≥60 years old; ②Have lived in urban communities in Fuzhou for ≥6 months; ③Have lived with family members; ④Have been diagnosed with hypertension or diabetes by secondary or above hospitals.
Family caregivers: ① Aged ≥ 18 years old; ② Willing to participate in this study and able to communicate normally; ③ Family members who live together with elderly patients with chronic diseases and undertake their main daily care work; ④ If the patient has several families at the same time The caregiver takes care of the family member with the longest caring time.

排除标准：

患者：听力障碍、言语障碍、认知障碍、精神疾病、重症或终末疾病的患者。
家庭照顾者：①存在认知或精神障碍者；②行动不便者。

Exclusion criteria：

Patients: Patients with hearing impairment, speech impairment, cognitive impairment, mental illness, severe or terminal illness.
Family caregivers: ① Those with cognitive or mental disorders; ② Those with limited mobility.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

由经过专门培训的资料收集员对数据进行测量和采集。采用Excel 2016建立数据库，为保证数据录入的准确性，所有数据均双人录入；采用SPSS软件进行数据处理。
