研究目的：

观察常规药物治疗基础上，辅助物理治疗及运动对下肢慢性痛风性关节炎关节功能、生活质量及代谢指标等的影响。  

Objectives of Study：

To observe the effects of assisted physical therapy and exercise on joint function, life quality and metabolic indicators of chronic gouty arthritis patients with conventional drug therapy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.慢性痛风关节炎患者（痛风诊断符合美国风湿病学会及欧洲抗风湿病联盟提出的痛风分类标准），合并肉眼痛风石，且影响下肢关节≤4个关节；
2.第一跖趾、踝、膝关节三部位中的任何一个关节存在慢性疼痛、关节角度或动作受限，导致日常生活受限者；
3.降尿酸药物为单用非布司他，不联合使用其他降尿酸药物者；
4.拥有并会使用智能手机；
5.获得受试者、或家属、或监护人、或法定代理人的同意并签署知情同意书。

Inclusion criteria

1.Diagnosed as chronic gouty arthritis, according to the 2015 ACR/ELUAR clinical classification of gout diagnostic criteria;
2.Aged between 18 and 70 years old;
3.With visible tophi affecting 0-4 joints of bilateral lower extremities;
4.Patients with chronic pain, limitations in range of motion and daily activities over 1st toe, ankle, or knee joints);
5.Use febuxostat as urate-lowering drug, instead of other urate-lowering drugs;
6.Use smart mobile phone.

排除标准：

1.重度及不稳定痛风石：痛风石生长迅速；痛风石导致神经受压、痛风石破溃或濒临破溃、痛风石破溃合并感染等；
2.慢性痛风性关节炎急性发作者；
3.患有蜂窝性组织炎、反复性风湿症(游走性关节炎)、细菌性关节炎、假性痛风、血清阴性脊椎关节炎、类风湿性关节炎、退化性关节炎等影响关节功能的疾病；
4.CKD ≥3期患者；
5.过去6个月内接受过下肢关节内药物注射或者是手术治疗的；
6.依从性差者，难以配合治疗者，（包括不遵医嘱口服/注射/外涂药物、其他物理治疗干预等）；
7.无法使用辅具独立行走者；
8.合并严重器质性疾病，比如血流动力学不稳定的心脏疾病、COPD、新近发生的脑血管疾病、未控制的高血压、糖尿病、恶性肿瘤等；
9.患有无法独立生活或配合治疗的精神疾病；
10.无法定期做物理治疗者。

Exclusion criteria：

1.Severe and unstable tophi:
(1)visible tophi affecting more than 4 joints of bilateral lower extremities;
(2)rapid growing tophi;
(3)nerve compression by tophi;
(4)connective tissue destructive effects;
(5)tophaceous joints inflammation.
2.Patients with secondary hyperuricemia caused by hematological diseases, malignant tumors, kidney diseases or radiotherapy and chemotherapy.
3.Chronic gouty arthritis with acute attack for 3 days.
4.Arthritis caused by cellulitis, palindromic rheumatism, bacterial arthritis, pseudogout, ankylosing spondylitis, rheumatoid arthritis, rheumatoid arthritis, or osteoarthritis.
5.Other diseases:
(1)Uncontrolled hypertension (resting blood pressure ≥160/100mmHg);
(2)Uncontrolled diabetes (fasting gluscose>7.1 mmol/l, HbA1C>7.0 %);
(3)Malignant tumor;
(4)Organic disease e.g. unstable and severe cardiac disease, heart failure, unstable angina, myocardial infarction diagnosed by cardiologist;
(5)Chronic obstructive pulmonary disease;
(6)Recent cardiovascular disease.
6.Patients with active gastrointestinal ulcer, thyroid dysfunction, active tuberculosis and urinary calculi.
7.Patients with active liver disease or cirrhosis, or liver dysfunction.
8.Chronic kidney disease ≥3 stages (eGFR < 60ml/min), according to the simplified MDRD formula, GFR (ml/min 1.73m2) = 186 * (Scr) - 1.154 * (age) - 0.203 * (0.742 women).
9.Received joints injection or surgery over lower extremities in past 6 months.
10.Poor compliance and have difficulties in taking medicine, receiving physical therapy once per week, doing exercises for 3 months.
11.Patients who cannot walk independently with assistive device.
12.Patients with psychological disease cannot live independently and cooperate treatment.
13.Alcoholism and use illegal drugs.
14.Pregnant, lactating women or prepare to pregnancy women (including male subjects).
15.Participated in other research trials in past 3 months before this trail.
