研究目的：

1.结合实际构建具有适老性、趣味性、团体性及心理治疗性的基于团体游戏疗法的老年糖尿病患者健康教育干预方案。 2.探讨基于团体游戏疗法的健康教育干预对老年糖尿病患者的自我管理行为、血糖水平、生活质量及抑郁状态等方面的影响。 3.为团体游戏疗法在糖尿病健康教育领域的应用和推广提供参考。  

Objectives of Study：

1. Based on the reality, construct an age-appropriate, interesting, group-based and psychotherapeutic health education intervention program for elderly diabetes patients based on group play therapy. 2. To explore the effects of health education intervention based on group play therapy on self-management behavior, blood glucose level, quality of life and depression of elderly diabetes patients. 3. To provide reference for the application and promotion of group play therapy in diabetes health education.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄：大于等于60周岁，包括60岁以前和60岁以后诊断糖尿病的老年人。
2.符合 WHO（1999 年）提出的糖尿病诊断标准，明确诊断为2型糖尿病，并纳入社区慢性病管理系统的患者；
3.生活能够自理，可以参加健康教育讲座；
4.能正常进行言语沟通，对教育内容有一定理解能力；
5.病情稳定，且未合并严重的冠心病、高血压、肝肾功能不全、肺功能不良以及骨关节疾病；
6.自愿参加本研究，预计一年内不会因出国、休假等原因中断参加本研究。

Inclusion criteria

1. Age: older than or equal to 60 years old, including those diagnosed with diabetes before and after 60 years old.
2. Patients WHO met the diagnostic criteria of diabetes proposed by WHO (1999) and were clearly diagnosed as type 2 diabetes and included in the community chronic disease management system;
3. Be able to take care of themselves and attend health education lectures;
4. Be able to communicate with others and understand educational content;
5. The condition is stable without serious coronary heart disease, hypertension, liver and kidney insufficiency, lung dysfunction or bone and joint diseases;
6. Voluntarily participate in the study, and it is expected that the study will not be interrupted by going abroad or taking a vacation within one year.

排除标准：

1.2型糖尿病诊断不明确的患者；
2.有认知功能障碍或生活不能自理、不易配合（智力、听力、肢体活动明显障碍）的患者；
3.文化程度低或智力理解能力较差，不能接受健康教育内容的患者；
4.伴有糖尿病严重并发症的患者，如糖尿病足或肢体残疾；
5.伴有严重心、脑、肾、肝及呼吸系统疾病或骨关节疾病，预期寿命不足两年。
6.同时参与其他研究项目的患者。

Exclusion criteria：

1.Patients with unclear diagnosis of type 2 diabetes;
2. Patients with cognitive dysfunction or inability to take care of themselves and not easy to cooperate (obvious impairment of intelligence, hearing and body movement);
3. Patients with low education level or poor intellectual understanding ability, who cannot accept health education content;
4. Patients with serious complications of diabetes, such as diabetic foot or limb disability;
5. With severe heart, brain, kidney, liver and respiratory diseases or bone and joint diseases, life expectancy is less than two years.
6. Patients who are also participating in other research projects.
