研究目的：

通过筛选多中心十年肾活检资料中符合微小病变肾病综合征不伴血尿的1岁~12岁患儿电镜标本为研究对象，应用Motic Med 6.0数码医学图像分析系统测定肾小球基底膜（GBM）厚度，以每岁年龄为一年龄组分层进行统计不同年龄组之间的差异，以确立基底膜不同年龄段的划分并建立每一年龄段的基底膜正常参考值范围，进一步对每一年龄段再进行性别分组了解性别之间差异及相关GBM数值，从而建立不同性别、年龄段儿童肾小球基底膜（GBM）厚度的正常参考值范围，拟建议儿童薄基底膜肾病（TBMN）的GBM弥漫变薄的标准，并在临床应用中进行初步验证。  

Objectives of Study：

In this study, electronic microscope specimens of children aged 1-12 years old who were in line with nephrotic syndrome without hematuria were selected from multi-center 10-year renal biopsy data,and use Motic Med 6.0 digital medical image analysis system to measure the thickness of glomerular basement membrane (GBM) .To establish the division of basement membrane in different age groups,we take every 1 year old as an age group to statistically analyze the differences among different age groups ,and in order to establish the normal reference range of basement membrane in each age group, and further divide each age group into gender groups to learn gender differences and related GBM values, so as to establish the normal reference range of GBM thickness in children of different genders and ages. It is proposed to recommend GBM diffuse thinning criteria for children with thin basement membrane nephropathy (TBMN) and conduct preliminary validation in clinical application.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1)、患者年龄在 1~12岁之间；
(2)、无家族遗传病史；
(3)、临床上表现为原发性肾病综合征或者大量蛋白尿，且不伴有任何程度的血尿；
(4)、均已行肾活检且肾脏病理符合：免疫荧光染色全部为阴性或仅有少量非特异性沉积( 如IgM或C3+~++ ) ，光镜检查肾小球未见异常，透射电镜下诊断为微小病变（MCD），即仅可见上皮足突广泛融合，基底膜无分层、无皱缩，不伴系膜增生；
(5)、任何部位均无电子致密物沉积；
(6)、电镜下可以观察到至少1个肾小球。

Inclusion criteria

(1)The age of patient is between 1 to 12 years old;
(2)There is no family history and hereditary disease;
(3)Clinically manifested as PNS or massive proteinuria without hematuria of any degree;
(4)Kidney biopsy has been performed and the kidney pathology has been conformed:immunity fluorescence staining is negative or there is only a small amount of non-specific deposits (such as IgM or C3+~++);there is no abnormalities of glomerulus in light microscopythe ,by transmission electron microscopy,it is diagnosed as minimal disease (MCD) , only extensive epithelial foot process fusion, no delamination or shrinkage of the basement membrane, no mesangial hyperplasia,;
(5)No electron dense depositing in any part;
(6)At least one glomerulus can be observed under the electron microscope.

排除标准：

电镜照片质量差，无法满足图像软件对测量的要求，再重新切片、摄片后仍不能测量者

Exclusion criteria：

The electron microscope photos are unqualified and cannot meet the requirements of the imaging software, and the measurement still cannot be performed after re-slicing and filming.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

在 2021年 12月 1 日到 2022年06月 30 日期间，从我院所接收的肾活检标本中按照规定的入排标准筛选合适病例，初步以每一岁为一个年龄段，根据文献查阅，收集样本110，最少不能低于20例，故设定每个年龄段至少20例。先从负责单位单中心测量数据绘制散点图，初步分析年龄趋势，再从其他合作中心补充，以年龄段分组，方差分析组间差异后再统计不同年龄段组之间的差异，建立基底膜不同年龄段的划分并建立每一年龄段的基底膜正常参考值范围

First,drawing scatterplot by measurement data from the responsible unit single center , analyze the trend of age preliminarily, as well as other cooperation centers' supplement by age group ,analyze the variance after the statistical difference between different age groups , so as to establish the division of basement membrane in different age groups,and establish the normal reference range of basement membrane in each age group
