研究目的：

1.通过测量健康志愿者和重症患者肌肉的弹性值，评估剪切波弹性成像在ICU-AW中的可行性。 2.通过监测ICU-AW和非ICU-AW患者肌肉的弹性值，探讨剪切波弹性成像在ICU-AW中的诊断价值，评估肌肉弹性值的变化能否早期预测ICU-AW的发生。 3.通过监测ICU-AW康复前后的弹性值，探讨剪切波弹性成像在评估康复疗效中的应用价值。  

Objectives of Study：

1. To evaluate the feasibility of shear wave elastography in ICU-AW by measuring the elastic values of muscles in healthy volunteers and critically ill patients. 2. To explore the diagnostic value of shear wave elastography in ICU-AW by monitoring the muscle elasticity values of ICU-AW and non-ICU-AW patients, and to evaluate whether changes in muscle elasticity values can predict the occurrence of ICU-AW early. 3. By monitoring the elasticity value before and after ICU-AW rehabilitation, the application value of shear wave elastography in evaluating the rehabilitation effect was discussed.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）入住我院重症医学科的所有患者
（2）入住 ICU 前接受机械通气持续时间＜24h或入院后接受机械通气者
（3）签署知情同意书

Inclusion criteria

(1) All patients admitted to the intensive care unit of our hospital
(2) Those who received mechanical ventilation for less than 24 hours before admission to ICU or received mechanical ventilation after admission
(3) Sign the informed consent form

排除标准：

（1）年龄<18岁
（2）孕妇及哺乳期妇女
（3）既往存在或入院时和诊疗过程中通过临床表现及相关检查证实存在急慢性神经肌肉疾病?
如格林巴利综合征、周围神经炎、急性脑血管疾病、颅内感染、颅内占位性病变、创伤性颅脑损伤、脊髓损伤、阿尔茨海默病、脑萎缩、认知功能障碍、精神疾病不能与医生合作及完善MRC评分者
（4）机械通气时间＜48小时
（5）在进行 ICU-AW 评估及骨骼肌超声检查前死亡、转院或放弃治疗者
（6）因患者个体因素或体位限制不能行超声检查的患者：如四肢肢体手术或骨折固定、肢体截肢或严重烧伤者、胸腔手术等
（7）患者同时参与另一个临床试验
（8）因为不可纠正的疾病状态，如恶性肿瘤晚期或其他疾病的终末期；或医师认为即将死亡的临终状态者
（9）需要肌松药物治疗者
（10）病态肥胖患者（体重指数(BMI)>45kg/m2)

Exclusion criteria：

(1) Age <18 years old
(2) Pregnant women and lactating women
(3) Acute or chronic neuromuscular disease confirmed by clinical manifestations and related examinations at the time of admission and during the diagnosis and treatment process.
Such as Guillain-Barré syndrome, peripheral neuritis, acute cerebrovascular disease, intracranial infection, intracranial space-occupying lesions, traumatic brain injury, spinal cord injury, Alzheimer's disease, brain atrophy, cognitive dysfunction, Mental Illness Inability to Collaborate with Physicians and Perfect MRC Scorers
(4) Mechanical ventilation time < 48 hours
(5) Those who died, were transferred or gave up treatment before ICU-AW assessment and skeletal muscle ultrasonography
(6) Patients who cannot undergo ultrasound examination due to individual factors or body position constraints: such as limb surgery or fracture fixation, limb amputation or severe burns, thoracic surgery, etc.
(7) The patient is participating in another clinical trial at the same time
(8) Due to an uncorrectable disease state, such as advanced malignant tumor or terminal stage of other diseases; or a terminal state that the physician believes is about to die
(9) Those who need muscle relaxant therapy
(10) Morbidly obese patients (body mass index (BMI)>45kg/m2)
