研究目的：

（1）胸骨旁肋间肌超声预测NIV结局的预测价值的价值； （2）肋间肌厚度变异度预测NIV结局的界值，敏感度与特异度。  

Objectives of Study：

(1) The predictive value of parastinal intercostal muscle ultrasound in predicting NIV outcome; (2) The threshold, sensitivity and specificity of the variation of intercostal muscle thickness in predicting NIV ending.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）2021年11月-2023年12月入住皖南医学院弋矶山医院重症医学科符合急性呼吸衰竭标准的患者，（1）呼吸频率≥35次/分，（2）在文丘里吸氧或经鼻高流量吸氧流速在60L/min条件下，氧合指数（PaO2/FiO2）≤200mmHg（1mmHg＝0.133kPa）；（3）年龄＞18岁。

Inclusion criteria

(1) patients with acute respiratory failure admitted to the icu of yijishan hospital, wannan medical college from December 2021 to December 2023, (1) respiratory rate ≥35 times /min, (2) under the condition of venturi oxygen inhalation or nasal high-flow oxygen inhalation at a flow rate of 60L/min, Oxygenation index (PaO2/FiO2) ≤200mmHg (1mmHg = 0.133kPa); (3) Age > 18.

排除标准：

（1）妊娠；（2）膈肌麻痹；（3）哮喘或COPD急性加重；（4）神经肌肉疾病；（5）中度肥胖身体指数≥35kg/m2；(6)咳痰无力或无气道保护能力患者；（7）需要紧急插管；（8）NIV不耐受。NIV 不耐受被定义为患者因不适而拒绝NIV；

Exclusion criteria：

(1) pregnancy; (2) diaphragmatic paralysis; (3) acute exacerbation of asthma or COPD; (4) neuromuscular diseases; (5) Moderately obese body index ≥35kg/m2; (6) Patients with weak expectoration or no airway protection ability; (7) Emergency intubation is required; NIV intolerance. NIV intolerance was defined as the refusal of NIV due to discomfort;
