研究目的：

研究脑室外引流术导致的手术部位感染（surgical site infection,SSI）如切口感染、EVD引流管创口感染、脑脓肿、脑膜炎等的发生率，并通过前瞻性、开放性研究观察比较抗生素浸渍导管(BACTISEAL EVD，B-EVD)与常规导管(EVD)在脑室外引流治疗中对于预防手术部位感染的意义。  

Objectives of Study：

The purpose of this study is to establish initial baseline surgical site infection (SSI)such as incision infection brain abscess and meningitis for the BACTISEAL External Ventricular Drainage (EVD) System (referred to hereafter as B EVD) and to compare relative rates of SSI between patients with BACTISEAL or conventional EVD catheters in a prospective observational open label study.

药物成份或治疗方案详述：

本研究是一个多中心前瞻性、开放性、观察性的对比研究。观察18岁-70岁年龄之间的因颅脑疾病需要接受首次脑室外引流术治疗的患者，在植入EVD导管后手术部位感染的发生情况。 总共计划入选和观察至少574例可评价的患者，需要保证可以评价的植入B-EVD和常规EVD的患者的数量各自不少于287例。 外引流管的植入标准：由患者的诊治医生依据患者的病情需要以及患者家属的意愿，对于需要进行脑室外引流治疗的患者，植入常规外引流管或抗生素浸渍外引流管。 

Description for medicine or protocol of treatment in detail：

This is a prospective observational open label study of 574 evaluated implanted subjects at up to 20 investigator sites. Observing the incidence of surgical site infection after EVD catheter implantation in patients between 18 and 70 years old who need the first external ventricular drainage for craniocerebral diseases. Conventional EVD or B-EVD should be implanted according to the patient's disease condition and the wishes of his/her family members. 

纳入标准：

纳入符合下列所有入选标准的患者:
1）18岁-70岁（包括18岁和70岁）的成年人。
2）患者由于颅脑疾病植入了常规EVD导管或B-EVD导管，而且预期将接受至少72小时的EVD系统外引流。
3）EVD导管植入时引流管皮下潜行距离大于或等于3cm，外引流系统按封闭系统护理，例如，只在需要时才开放滴定管与引流袋之间的连接。

Inclusion criteria

Subject may be included in the stud y only if they meet all of the following criteria:
1 The subject is 18-70 years.
2 The subject is expected to receive EVD draining for at least 72 hours.
3 When EVD catheter was implanted, the subcutaneous distance of drainage tube was more than or equal to 3cm, and the external drainage system was nursing according to closed system, i.e. burette only opened to drainage bag as needed.

排除标准：

排除符合下列任何排除标准的患者:
1）参加本研究项目前30天内，患者参加过了使用抗生素药物、医疗器械植入、直接影响脑脊液的操作和/或研究项目。
2）已知的或怀疑的对利福平、克林霉素、硅胶成分或其它引流管组成成分过敏的患者。
3）在EVD导管植入前24小时内或植入时采集到的脑脊液标本细菌培养阳性。
4）患者有免疫抑制。
5）患者需要同时植入一根以上的外引流导管。
6）患者在本研究前30天内植入过外引流导管。
7）患者患有脓毒血症、或脑膜炎、或穿刺部位/附近的皮肤感染/炎症、或一侧或双侧的耳部感染、或严重的呼吸道感染，以及其它研究者认为对EVD系统作用有影响的感染性疾病。
8）患者在EVD植入前14天内接受全身系统的抗生素治疗，但不包括在EVD导管植入前48小时内进行的术前预防性抗生素治疗。
9）患者有未能控制的凝血功能障碍、或者其它出血性体质。
10）利用EVD系统进行灌洗治疗者。
11）合并有脑脊液漏的病人。
12）患者患有其它研究者认为不适合参加本研究项目的医学疾病。

Exclusion criteria：

Subjects will be excluded from the study for any of the following reasons:
1 The Subject has enrolled or participated in antibiotics, or medical device implantation, and operations that directly affect the cerebrospinal fluid and / or spinal cord device trial within preceding 30 days.
2 The Subject has a suspected or documented allergy to the materials of the EVD system, including silicone, rifampin and clindamycin.
3 The Subject has a positive CSF culture prior to EVD implant.
4 The Subject is immunocompromised.
5 The subject requires more than one ventricular catheter concurrently.
6 The Subject has had an EVD catheter within the previous 30 days.
7 The Subject has sepsis, ventriculitis, meningitis, a skin infection/inflammation at or near the implantation site, and ear infection on ei ther side, or a severe respiratory tract infection that, in the opinion of the investigator, would most likely compromise the effectiveness of the EVD system.
8 The Subject is known to have received systemic antibiotic therapy within a 14 day period
preced ing screening for inclusion in this study.
9 The subject has an uncontrolled coagulopathy or any other know bleeding diathesis.
