研究目的：

我国老龄化趋势增长显著，髋部骨折发生率年平均增长1.3％，而其合并内科疾病的比例高达55.8％。因此，髋部骨折患者的麻醉风险之高可想而知。外周神经阻滞已被专家们认为是一种安全、有效的镇痛方式或者全麻辅助手段。它不仅大大减少了麻醉过程中药物的使用量，还可以减少术后24小时内镇痛药物的应用，从而降低患者围术期并发症的发生率。髂筋膜间隙阻滞可有效阻滞股外侧皮神经和股神经，但是它对闭孔神经的阻滞效果仍存在争议。髂腰肌平面阻滞是一种有效缓解髋部骨折疼痛的新兴技术，其优势是可以精准阻滞股神经感觉分支而不影响髋关节运动能力。本课题通过比较髂腰肌平面阻滞与髂筋膜间隙阻滞在髋部手术中的效果，为患者围术期镇痛提供参考。  

Objectives of Study：

The aging trend in China is growing significantly, and the incidence of hip fracture is increasing by an average of 1.3% per year, while the percentage of their combined medical diseases is as high as 55.8%. Therefore, the risk of anesthesia in patients with hip fractures is conceivably high. Peripheral nerve block has been recognized by experts as a safe and effective analgesic modality or adjunct to general anesthesia. It not only greatly reduces the amount of drugs used during anesthesia, but also reduces the application of analgesic drugs in the 24 hours after surgery, thus reducing the incidence of perioperative complications in patients. The iliopsoas plane block can effectively block the lateral femoral cutaneous and femoral nerves, but its effectiveness on the closed foraminal nerve remains controversial. Iliopsoas plane block is an emerging technique to effectively relieve hip fracture pain, with the advantage of precisely blocking the sensory branches of the femoral nerve without affecting hip motion. In this study, we compared the effectiveness of iliopsoas plane block and iliac fascia gap block in hip surgery to provide reference for patients' perioperative analgesia.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）美国麻醉医师协会（ASA）分级为Ⅰ～Ⅲ级；（2）年龄18～95岁；（3）择期全身麻醉下行单侧髋部手术患者。

Inclusion criteria

(1) ASA classification I-III; (2) age 18-95 years; (3) patients undergoing unilateral hip surgery under elective general anesthesia.

排除标准：

（1）严重心肺脑合并症；（2）凝血功能异常；（3）局麻药物过敏；（4）穿刺部位感染；（5）合并精神疾病；（6）患者及家属拒绝；（7）手术时间超过2小时。

Exclusion criteria：

(1) severe cardiopulmonary and cerebral comorbidities; (2) abnormal coagulation function; (3) allergy to local anesthetic drugs; (4) infection at the puncture site; (5) combined mental illness; (6) refusal by the patient and family; (7) operation time more than 2 hours.
