1. 建立老年衰弱人群诊断和评估标准； 2. 评估老年衰弱人群危险因素； 3. 老年慢病对衰弱患者预后影响； 4. 衰弱对老年慢病患者不良预后影响； 5. 探索可穿戴设备在衰弱诊断中的应用。  

Objectives of Study：

1. Establishment of diagnostic and evaluation criteria for frailty; 2. To evaluate the risk factors of frailty; 3. The influence of chronic diseases on the frailty; 4. Effect of frailty on prognosis of elderly patients with chronic diseases. 5. Evaluate the effect of chronic diseases on the changes of body function and cognitive function in the elderly. 6. Explore the application of wearable equipment in the diagnosis of frailty.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄 ≥65岁；
2.患以下一种或一种以上疾病：
1）高血压病；
2）冠状动脉粥样硬化性心脏病；
3）2型糖尿病；
4) 慢性阻塞性肺部疾病；
5）脑卒中。

Inclusion criteria

1. More than 65 years old;
2. To suffer from one or more of the following diseases:
1) Hypertension;
2) Coronary artery disease;
3) Type 2 diabetes mellitus;
4) Chronic obstructive pulmonary disease;
5) Stroke.

排除标准：

1. 重症和终末疾病患者，预期寿命<1年；
2. 有重大残疾或精神疾患、严重认知障碍、自主活动障碍等不能配合完成衰弱评估者；
3. 恶性肿瘤病史。

Exclusion criteria：

1. Limited life expectancy（less than 1 year） due to severe or end-stage diseases;
2. Major disability or mental illness,Severe cognitive impairment, limited independent activity that cannot cooperate to complete evaluation;
3. History of malignant tumors.
