研究目的：

全球慢性肾脏病(chronic kidney disease，CKD)患病率约14.3%，中国CKD患病率约10.8%。随着肾功能的下降，CKD患者营养不良风险增加、心肺耐力下降、肌肉萎缩，生活质量显著降低，心血管事件和死亡风险显著升高。已有大量临床研究证实，运动治疗与药物治疗一样，对改善肾病患者的整体预后有着重要意义，治疗方案也有基于循证医学证据的临床实践指南。但传统运动干预方式成本高、效率低、患者依从性差，按慢性肾病诊疗指南接受了运动治疗的患者比例可能不足1%。寻找创新、高效、规范的肾病综合管理办法，对提升慢性肾病整体预后的十分重要。 本实验旨在了解我院随访的慢性肾脏病患者的心肺功能、肌少症、炎症状态、生活质量状况，运用数字化软件分别予以个性化运动康复处方，验证远程监控下运动在慢性肾脏病运动康复中应用的可行性；研究数字化远程监控下运动康复在慢性肾脏病管理中的作用。  

Objectives of Study：

The global prevalence of chronic kidney disease (CKD) is about 14.3%, and that of CKD in China is about 10.8%. With decreased renal function, CKD patients are at increased risk of malnutrition, decreased cardiopulmonary endurance, muscle atrophy, significantly reduced quality of life, and significantly increased risk of cardiovascular events and death. A large number of clinical studies have demonstrated that exercise therapy is as important as drug therapy in improving the overall prognosis of patients with kidney disease, and there are evidence-based clinical practice guidelines for treatment options. However, due to the high cost, low efficiency and poor patient compliance of traditional exercise intervention methods, the proportion of patients receiving exercise therapy according to the guidelines for the diagnosis and treatment of chronic kidney disease may be less than 1%. It is very important to find innovative, efficient and standardized comprehensive management of nephropathy to improve the overall prognosis of chronic kidney disease. The purpose of this trial is to explore the cardiopulmonary function, sarcosis, inflammatory state and quality of life of patients with chronic kidney disease followed up in our hospital, and use digital software to give personalized exercise rehabilitation prescription respectively, to verify the feasibility of remote monitoring exercise in the exercise rehabilitation of chronic kidney disease. To study the effect of exercise rehabilitation under digital remote monitoring in the management of chronic kidney disease.

纳入标准：

①有慢性肾脏病病史≥3 个月，或规律透析（血液透析/腹膜透析）治疗≥3 个月；
②遵医嘱规律随访者；
③年龄≥18岁；
④自愿签署知情同意书；
⑤拥有并会使用智能手机的患者

Inclusion criteria

① Have a history of chronic kidney disease ≥3 months, or regular dialysis (hemodialysis/peritoneal dialysis) treatment ≥3 months;
② Follow the doctor's advice;
③ Age ≥18 years;
④ Voluntarily sign informed consent;
⑤ Patients who own and use smart phones

排除标准：

（满足以下标准之一即排除）
①未控制的血压异常：严重的高血压（如血压超过 180/110 mmHg），或低血压（＜90/60 mmHg）；
②严重心肺疾病：严重的心力衰竭、心律失常，不稳定性心绞痛，重度心包积液、瓣膜狭窄，肥厚性心肌病，主动脉夹层等，未控制的肺动脉高压（肺动脉平均压＞55 mmHg）；
③急性临床事件：如急性全身炎症性疾病、电解质紊乱等；
④深静脉血栓的症状，如小腿不正常的水肿、发红和疼痛时要暂缓或停止运动；
⑤严重水肿、骨关节病等不能配合运动等；
⑥患有无法参加运动的神经肌肉骨骼疾病；
⑦患有无法独立生活或配合治疗的精神疾病；
⑧不配合治疗（包括不遵医嘱口服/外涂/注射药物、运动干预、饮食控制等）；
⑨有接触性皮炎病史者；
⑩未签署知情同意书。

Exclusion criteria：

(One of the following criteria is excluded)
① Uncontrolled abnormal blood pressure: severe hypertension (such as blood pressure over 180/110 mmHg), or hypotension (< 90/60 mmHg);
② Serious heart and lung diseases: serious heart failure, arrhythmia, unstable angina pectoris, severe pericardial effusion, valve stenosis, hypertrophic cardiomyopathy, aortic dissection, uncontrolled pulmonary hypertension (mean pulmonary arterial pressure > 55 mmHg);
③ Acute clinical events: such as acute systemic inflammatory diseases, electrolyte disorders, etc.;
④ Symptoms of deep vein thrombosis, such as abnormal edema, redness and pain in the leg to suspend or stop movement;
⑤ severe edema, bone and joint diseases can not cooperate with sports, etc.;
⑥ Have neuromusculoskeletal diseases that cannot participate in sports
⑦ Suffering from mental illness that cannot live independently or cooperate with treatment;
⑧ do not cooperate with treatment (including oral/external coating/injection drugs, exercise intervention, diet control, etc.);
⑨ Persons with a history of contact dermatitis;
⑩ Informed consent was not signed.
