研究目的：

建立中晚期咽喉癌精准诊治的应用体系。利用高标准生物样本库和大数据平台，整合基因组学、转录组学、蛋白组学等多组学数据，创建咽喉癌诱导化疗敏感性预测模型和预后分子生物学分型，开展多中心临床研究，完成一系列精准医学整合体系的临床转化研究。从而提升中晚期咽喉癌的综合诊治能力，开发新型诊疗模式，最终提高中晚期咽喉癌患者的总体生存率和喉功能保留率。  

Objectives of Study：

To establish an application system for accurate diagnosis and treatment of middle and advanced pharyngeal cancer. High-standard biobank and big data platforms were utilized to integrate multiple omics data, such as genomics, transcriptomic, and proteomics, to establish sensitivity prediction model and prognostic molecular biotyping for induction chemotherapy of pharyngeal cancer, carry out multi-center clinical studies and complete a series of clinical translational studies of the precision medicine integration system. In order to improve the comprehensive diagnosis and treatment ability of middle-advanced laryngeal cancer, develop a new diagnosis and treatment model, and ultimately improve the overall survival rate and laryngeal function retention rate of patients with middle-advanced laryngeal cancer.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①中晚期咽喉癌患者（局部肿瘤T2-4a，任何N）② 年龄18-75岁；③ 初治患者（原发，无手术、放疗及化疗病史）；④ KPS 评分80 分及以上；⑤ 血象、心、肺、肝、肾等主要器官功能基本正常。WBC≥3.5×109/L，ANC≥1.5×109/L ，PLT≥100×109/L，Hb≥80g/L，血浆总胆红素≤1.5×正常值上限，ALT、AST 或AKP≤2.5×正常值上限，尿素氮、肌酐≤1.25×正常值上限；⑥ 入组前完善颈部及胸部增强CT、内镜检查；⑦ 入组前病理学检查（分化程度、免疫学指标-Ki-67，EGFR，HPV）；⑧ 签署知情同意书，依从性好。

Inclusion criteria

① Patients with intermediate and advanced laryngeal cancer (local tumor T2-4A, any N) ② age 18-75 years; ③ Newly treated patients (primary disease, no history of surgery, radiotherapy or chemotherapy); ④ KPS score of 80 or above; ⑤ The functions of blood, heart, lung, liver, kidney and other major organs were basically normal. WBC≥3.5×109/L, ANC≥1.5×109/L, PLT≥100×109/L, Hb≥80g/L, plasma total bilirubin ≤1.5× normal upper limit, ALT, AST or AKP≤2.5× normal upper limit, urea nitrogen, creatinine ≤1.25× normal upper limit; ⑥ Enhanced CT and endoscopy of neck and chest were completed before enrollment; ⑦ Pathological examination before enrollment (differentiation degree, immunological indicators - KI-67, EGFR, HPV); ⑧ Signed informed consent with good compliance.

排除标准：

① 远处转移患者；②可通过支撑喉镜下激光等微创手术切除的患者；③有不可控制的癫痫发作、中枢神经系统紊乱或因精神病丧失自知力者；④ 严重不可控制的感染；⑤ 主要脏器功能不全者（包括心、肺、肝、肾等）；⑥无法耐受全麻手术者；⑦ 同时或两年内有其它恶性肿瘤者。

Exclusion criteria：

① Patients with distant metastasis; (2) patients who can be removed by minimally invasive surgery such as laser under the support laryngoscope; ③ Have uncontrollable epileptic seizures, central nervous system disorder or loss of self-awareness due to mental illness; ④ Serious and uncontrollable infection; ⑤ Major organ dysfunction (including heart, lung, liver, kidney, etc.); ⑥ Can not tolerate general anesthesia surgery; ⑦ Having other malignant tumors at the same time or within two years.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

（1）临床标本收集：术中专人负责留取癌组织，专人负责收集与管理标本，每月举行科研例会向负责人进行汇报，确保组织标本充分及时有效留取。 （2）临床数据采集：建立标准操作规程，对数据管理人员进行培训，并定期进行数据收集的质量控制和检查。完善数据复核，发现错误后及时更正。存档保存收集好的临床数据。
