研究目的：

观察益生菌是否具有预防呼吸机相关性肺炎的作用  

Objectives of Study：

evaluate the preventive effect of probiotics on the incidence of ventilator associated pneumonia

药物成份或治疗方案详述：

益生菌制剂 美常安（枯草杆菌、肠球菌二联活菌） 

Description for medicine or protocol of treatment in detail：

probiotic capsula Meichangan (Bacillus Subtilis and Enterococcus faecalis) 

纳入标准：

年龄≥18岁；入住ICU时需要机械通气的患者，且估计时间≥48h

Inclusion criteria

Adult critically illed patients (>=18 years) with an expected need of mechanical ventilation for at least 48h

排除标准：

① 年龄＜18岁或>80岁；
② 严重多器官功能障碍者（APACHEⅡ评分≥30分）；
③ 转入ICU前已行超过72h机械通气的患者；
④ 需要禁食的患者。
⑤ 入组前2周使用过免疫抑制剂或存在免疫抑制性疾病，如艾滋病、HIV携带者；
⑥ 妊娠或哺乳期妇女。

Exclusion criteria：

(1) age < 18 years or >80 years;
(2) severe multiple organ failure, APACHE II scores > or = 30;
(3) patients who were mechanically ventilated more than 72 hours before admission to ICU;
(4) enteral feeding can not start;
(5) orally administration of immunodepressants a week before admission or diagnosed with immune supressed diseases, such as maglinant tumor, AIDS, HIV carriers;
(6) pregnancy or lactation.
