研究目的：

1.超声观测拔管后膈肌运动幅度、膈肌厚度变化率及恢复率与术后患者氧合指标的相关性； 2.评估运用膈肌超声改善全身麻醉拔管后患者术后氧合功能的可行性。  

Objectives of Study：

1. To observe the correlation between the range of motion of the diaphragm, the rate of change of the thickness of the diaphragm and the recovery rate after extubation after extubation and the oxygenation indexes of postoperative patients; 2. To evaluate the feasibility of using diaphragm ultrasound to improve postoperative oxygenation function in patients after general anesthesia and extubation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患者性别不限，ASAⅡ-Ⅲ级，年龄大于65岁，行Trendelenburg体位的手术 ，所有患者术前均神志清晰,相关检验检查基本正常。

Inclusion criteria

There was no limitation in gender, ASA II - III, aged > 65 years, and Trendelenburg position surgery was performed. All patients were conscious before surgery, and relevant examinations were basically normal.

排除标准：

1.既往有呼吸系统疾病史，既往心肺及胸部手术病史，胸廓畸形，胸腹腔积液或胸腔巨大肿瘤等；
2.合并严重心血管疾病的患者，如血压大于180/110mmHg，三个月心绞痛及心梗患者；
3.肝肾功能不全和精神疾病，神经肌肉病史，恶性高热家族史，视力、听力障碍无法配合的患者；
4.语言沟通障碍以及拒绝参与实验患者。

Exclusion criteria：

1.Previous history of respiratory disease, previous history of cardiopulmonary and chest surgery, thoracic malformation, pleural and abdominal effusion or large thoracic tumor;
2.Patients with serious cardiovascular disease, such as blood pressure greater than 180/110mmHg, angina pectoris and myocardial infarction at 3 months;
3.Patients with liver and kidney dysfunction and mental illness, neuromuscular history, family history of malignant high fever, vision and hearing impairment;
4.Verbal communication problems and refusal to participate in the experiment.
