研究目的：

主要目的：探究PRP预防食管大面积早癌ESD术后狭窄的有效性及安全性； 次要目的：探究PRP对于早期食管癌术后患者远期预后的影响。  

Objectives of Study：

Main purpose: to explore the effectiveness and safety of PRP in preventing postoperative stenosis of large area early esophageal cancer after ESD; Secondary purpose: to explore the effect of PRP on the long-term prognosis of patients with early esophageal cancer after surgery.

药物成份或治疗方案详述：

试验组：ESD术后立即于残余黏膜下层注射PRP，术后口服醋酸泼尼及质子泵抑制剂PPI治疗8周。 对照组：ESD术后立即于残余黏膜下层注射曲安奈德，术后口服醋酸泼尼及质子泵抑制剂PPI抑酸治疗8周。 PRP为由自体血液经提取离心制得，具有调节细胞的增殖、分化、血管生成和趋化的作用，还可以增加细胞膜的通透性，调节炎症过程。 

Description for medicine or protocol of treatment in detail：

Test group: PRP was injected into the residual submucosa immediately after ESD and treated with oral prednisolone acetate and proton pump inhibitor PPI for 8 weeks after surgery. Control group: Tretinoin was injected into the residual submucosa immediately after ESD, and postoperative oral treatment with prednisolone acetate and proton pump inhibitor PPI for 8 weeks. PRP is made from autologous blood by extraction and centrifugation, which has the effect of regulating cell proliferation, differentiation, angiogenesis and chemotaxis, and also increases cell membrane permeability and regulates the inflammatory process. 

纳入标准：

1.经病理检查或内镜精查考虑为食管早癌，病变面积占环周黏膜>=75%；ESD术后创面黏膜缺损>=75%；
2.年龄18~80岁；
3.治疗依从性良好，能完成随访者；
4.经影像学证实无远处转移及周围侵犯；
5.患者及家属均对本研究知情同意，签署知情同意书。

Inclusion criteria

1.It is considered to be early esophageal cancer by pathological examination or endoscopy, and the lesion area accounts for >=75% of the surrounding mucosa; the wound mucosal defect after ESD is >=75%;
2.Aged 18 to 80 years;
3.Good treatment compliance, can complete the follow-up;
4.No distant metastasis and surrounding invasion confirmed by imaging;
5.Patients and their families gave informed consent to this study and signed the informed consent form.

排除标准：

1.合并其他恶性肿瘤者或其他食管疾病者；
2.合并器官功能障碍者；
3.有控制不佳的糖尿病、股骨头坏死、消化性溃疡和肺结核；
4.妊娠或哺乳期妇女；
5.存在内镜手术禁忌者。如既往长期口服全身皮质醇激素及抗凝抗血小板药物。

Exclusion criteria：

1.Patients with other malignant tumors or other esophageal diseases;
2.Complicated with organ dysfunction;
3.Poorly controlled diabetes, necrosis of the femoral head, peptic ulcer and tuberculosis;
4.Pregnant or lactating women;
5.Patients with contraindications to endoscopic surgery. Long-term oral administration of systemic cortisol hormones and anticoagulant and antiplatelet drugs.
