探索诊断胃食管反流病的精准新方法和标准  

Objectives of Study：

To explore a new accurate method and standard for the diagnosis of gastroesophageal reflux disease

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）年龄在18~70岁之间，性别不限;
（2）病例组为具有胃食管反流病症状的患者；无胃食管反流病症状的人群作为对照组；
（3）自愿签署知情同意。

Inclusion criteria

1. Aged 18 to 70 years old male and female;
2. The case group was diagnosed as gastroesophageal reflux patient;the health control group was not suffered from stomach and esophageal symptoms and history of digestive tract diseases;
3. Willing to Sign the informed consent.

排除标准：

①有报警症状者（如进行性吞咽困难、吞咽疼痛、体重减轻、贫血、呕血和黑便等等）；
②胃镜检查疑有恶性病变；
③合并有消化道溃疡、胃手术史者；
④妊娠或哺乳期妇女；
⑤严重心、肝、肾疾病者；
⑥研究者认为不宜参加此项研究的其他原因。

Exclusion criteria：

1. Patients with alarm symptoms (e.g. progressive dysphagia, weight loss, anemia, hemoptysis, black stool, etc.);
2. Patients suspected malignant lesions by Gastroscopy;
3. Patients with history of gastrointestinal ulcer and gastric surgery;
4. Pregnant or lactating women;
5. Patients with severe heart, liver and kidney diseases;
6. Othere reasons that researchers consider for not taking part in the study.

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

胃镜检查为反流性食管炎诊断：分级参照Los Angeles标准: 正常为食管粘膜无破损；A级为病灶局限于食管黏膜皱襞，有1个或1个以上黏膜破损，长度≤5mm；B级为病灶仍局限于食管黏膜皱襞,仍是1个或1个以上黏膜破损，但长度>5mm，相互不融合；C级为病灶在黏膜顶部相融合，但不环绕整个食管壁（<75%食管壁）；D级为病灶相融合,且范围> 75%的食管壁。 胃镜检查为Barrett（BE）食管诊断：内镜检查正常时，鳞柱状上皮交界处（SCJ）在食管下段形成界限清楚的齿状线即“Z线”与内镜下胃食管交界标志（EGJ）一致。发生BE时Z线上移，遗留柱状上皮或Z线上方出现的柱状上皮黏膜，内镜分为全周型、岛型和舌型。长短BE为化生上皮累及食管全周且长度≥3cm，短段BE为化生上皮未累及食管全周或累及食管全周，但长度＜3cm。病理组织分型为a贲门腺型、b胃底腺型、c特殊肠化生型，可伴有异型增生。 胃镜检查为非糜烂性胃食管反流病（NERD）：临床表现为烧心、反酸及食管外表现，如胸骨疼痛、咳嗽、哮喘等（即有明显的胃食管反流症状），但胃镜检查无异常。

胃食管反流病超声造影检测评价体系: ①造影剂：声诺维（六氟化硫）、浙江省湖州东亚胃肠超声所生产的“天下牌”速溶胃肠超声助显剂； ②彩色超声诊断仪：飞利浦iu22，百胜Twice超声检查仪； ③探头：腹部探头、探头频率：1-5MHz。
