探究膈神经阻滞对射频消融术中及术后镇痛的效果为肝 脏肿瘤患者加速康复（ERAS）提供依据及方案  

Objectives of Study：

To explore the effects of phrenic nerve block on intraoperative and postoperative analgesia of radiofrequency ablation will provide the basis and plan for accelerated recovery of patients with liver tumors (ERAS)

药物成份或治疗方案详述：

2022 年 05月至 2023 年 06 月华中科技大学同济医学院 附属协和医院及浙江大学医学院附属第一医院医院，经诊 断为肝细胞癌（Hepatocellular Carcinoma, HCC）/肝脏 血管瘤（Hepatic hemangioma，HH）并计划行射频消融术 治疗并计划在静脉全身麻醉的条件下行射频消融术治疗 的肝肿瘤患者，针对研究对象进行前瞻性队列研究。 参加本研究的研究对象共 132 例，研究对象年龄为≥18 周岁(含 18 周岁)，男女 不限。经筛选合格的研究对象， 将被入组到本研究中，然后由医生和研究对象共同决定治 疗方案，根据治疗方案进入相应的队列，具体如下： 实验组（全身麻醉联合超声引导的膈神经阻滞组）：行超 声引导的膈神经阻滞联合静脉全身麻醉进行射频消融术 治疗肝癌的患者； 对照组（全身麻醉组）：行静脉全身麻醉进行射频消融术 治疗肝癌的患者； 签署知情同意书，且符合本研究所有 入选排除标准的患者将被纳入到本研究中，并将分别在筛 选期、射频消融手术期、术后 48 小时内 进行相应的随访 和临床信息的收集。 

Description for medicine or protocol of treatment in detail：

May 2022 to June 2023 Union Hospital affiliated to Tongji Medical College of Huazhong University of Science and Technology and the First Affiliated Hospital of Zhejiang University Medical College Patients diagnosed with Hepatocellular Carcinoma (HCC)/Hepatic hemangioma (HH) and planned to be treated with radiofrequency ablation under intravenous general anesthesia. Prospective cohort studies were conducted on the subjects. A total of 132 subjects were enrolled in this study, aged ≥18 years old (inclusive), male and female. Qualified subjects will be enrolled in this study, and then doctors and subjects will jointly determine the treatment plan and enter the corresponding cohort according to the treatment plan, as follows: Experimental group (general anesthesia combined with ultrasound-guided phrenic nerve block group) : Patients undergoing radiofrequency ablation with ultrasound-guided phrenic nerve block combined with intravenous general anesthesia for hepatocellular carcinoma; Control group (general anesthesia group) : patients receiving intravenous general anesthesia for radiofrequency ablation for hepatocellular carcinoma; Patients who signed informed consent and met all inclusion and exclusion criteria of the study will be enrolled in the study, and will be followed up and clinical information will be collected during screening, radiofrequency ablation, and 48 hours after surgery, respectively. 

纳入标准：

1) 年龄≥18 岁，性别不限； 2) 诊断为 HCC 并计划行射频消融术治疗的肝癌患者； 3) 仰卧位血压（进入手术室后 5 分钟）90-160/50-100mmHg； 4) 美国麻醉医师协会（ASA）I-III 级 5) Child-Plug 评分 A 级； 6) 无已知对所用药物过敏； 7) 愿意接受量表调查以及访问的人群。

Inclusion criteria

1) Age ≥18, no gender limitation; 2) HCC patients diagnosed with HCC and scheduled for radiofrequency ablation; 3) Blood pressure in supine position (5 minutes after entering the operating room) 90-160/50-100mmHg; 4) American Society of Anesthesiologists (ASA) Level I-III 5) Child-PLUG Score Level A; 6) No known allergy to the drug used; 7) People who are willing to accept the scale survey and interview.

排除标准：

1) 任何心电图（ECG）检查异常； 2) QTc 心电图异常（心率校正 Q 和 T 峰值间隔大于 450 ms）或其他心 脏危险因素，如心力衰竭、低钾血症或家族 长 QT 综合征病史； 3) 门静脉高压症；4) 中度至重度阻塞或肺功能受限； 5) 静息心率低于每分钟 40 次或高于 100 次； 6) 慢性疼痛、经常使用镇痛药、严重的肋骨畸形； 7) 对麻醉药物过敏。

Exclusion criteria：

1) Any abnormal electrocardiogram (ECG) examination; 2) Abnormal QTc electrocardiogram (peak interval between corrected Q and T greater than 450 ms) or other cardiac risk factors, such as heart failure, hypokalemia, or family history of long QT syndrome; 3) portal hypertension; 4) Moderate to severe obstruction or limitation of lung function; 5) Resting heart rate below 40 beats per minute or above 100 beats per minute; 6) Chronic pain, frequent use of analgesics, severe rib deformity; 7) Allergic to narcotic drugs.
