研究目的：

1.评估保肾方治疗2型糖尿病肾病（diabetic nephropathy，DN）IV期，属气阴两虚，瘀血阻络证患者的临床疗效和安全性。 2.通过观察高糖记忆标志物、足细胞调控因子、miR-21以及足细胞上皮-间充质转分化（epithelial-mesenchymal transition，EMT）相关蛋白等指标，阐明其作用机制。  

Objectives of Study：

1. To evaluate the clinical efficacy and safety of Baoshen prescription in the treatment of type 2 diabetic nephropathy (DN) stage IV, which belongs to the deficiency of both qi and yin and the syndrome of blood stasis blocking collaterals. 2. To clarify the mechanism of action by observing high glucose memory markers, podocyte regulatory factors, miR-21 and podocyte epithelial-mesenchymal transition (EMT) related proteins.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.符合T2DM诊断的患者；
2.符合西医糖尿病肾病Ⅳ期诊断标准并且合并糖尿病视网膜病变患者，而且尿蛋白≤3.5g/24h，血浆白蛋白≥30g/L，糖化血红蛋白＜8%，血肌酐＜3mg/dl；
3.符合气阴两虚，瘀血阻络证的患者；
4.年龄在18至80岁之间。

Inclusion criteria

1. Patients who meet the diagnosis of T2DM;
2. Patients who meet the diagnostic criteria of western medicine for diabetic nephropathy stage IV and are combined with diabetic retinopathy, and urine protein <=3.5g/24h, plasma albumin >=30g/L, glycosylated hemoglobin <8%, serum creatinine <3mg/dl;
3. Patients with deficiency of both qi and yin and blood stasis blocking collaterals;
4. Aged between 18 and 80 years.

排除标准：

1.妊娠或准备妊娠及哺乳期妇女；
2.合并心、脑、肺、肝和造血系统等严重原发性疾病，过敏体质者；
3.无法合作者，如精神病患者；
4.存在活动性感染灶：如肺结核等；
5.各种肿瘤继发肾脏病患者及结核病等消耗性疾病患者；
6.正在参加其它药物临床试验者或12周内参加过其它临床试验者；
7.12周内用过糖皮质激素、免疫抑制剂、雷公藤制剂。

Exclusion criteria：

1. Pregnant or planning to become pregnant and lactating patients;
2. Patients with serious primary diseases such as heart, brain, lung, liver and hematopoietic system, allergic constitution;
3. Unable to cooperate, such as mentally ill;
4. There are active infection foci: such as tuberculosis;
5. Patients with renal disease secondary to various tumors and patients with wasting diseases such as tuberculosis;
6. Those who are participating in clinical trials of other drugs or who have participated in other clinical trials within 12 weeks;
7. Glucocorticoids, immunosuppressants, and Tripterygium wilfordii preparations have been used within 12 weeks.

随机化方案借助SAS9.4统计分析软件产生2型糖尿病肾病（Ⅳ期）受试者所接受（试验组和对照组）的随机安排，即列出流水号001~094所对应的治疗分配。

The randomized scheme was used to generate the randomized arrangement (trial group and control group) of subjects with type 2 diabetic nephropathy (stage IV) with the help of sas9.4 statistical analysis software, that is, the treatment allocation corresponding to the serial number 001 ~ 094 was listed.
