研究目的：

主要目的是探究PRP预防食管大面积早癌ESD术后狭窄的有效性及安全性，次要目的是探究PRP对于早期食管癌术后患者远期预后的影响。  

Objectives of Study：

The main purpose was to explore the effectiveness and safety of PRP in preventing postoperative stenosis of large area early esophageal cancer after ESD, and the secondary purpose was to explore the impact of PRP on the long-term prognosis of patients with early esophageal cancer after ESD.

药物成份或治疗方案详述：

试验组：ESD术后立即于残余黏膜下层注射PRP，术后口服醋酸泼尼及质子泵抑制剂PPI治疗8周。 对照组：ESD术后立即于残余黏膜下层注射曲安奈德，术后口服醋酸泼尼及质子泵抑制剂PPI抑酸治疗8周。 PRP为由自体血液经提取离心制得，具有调节细胞的增殖、分化、血管生成和趋化的作用，还可以增加细胞膜的通透性，调节炎症过程。 

Description for medicine or protocol of treatment in detail：

Test group: PRP was injected into the residual submucosa immediately after ESD and treated with oral prednisolone acetate and proton pump inhibitor PPI for 8 weeks after surgery. Control group: Tretinoin was injected into the residual submucosa immediately after ESD, and postoperative oral treatment with prednisolone acetate and proton pump inhibitor PPI for 8 weeks. PRP is made from autologous blood by extraction and centrifugation, which has the effect of regulating cell proliferation, differentiation, angiogenesis and chemotaxis, and also increases cell membrane permeability and regulates the inflammatory process. 

纳入标准：

①经病理检查或内镜精查考虑为食管早癌，病变面积占环周黏膜≥75%；ESD术后创面黏膜缺损≥75%；
②年龄18~80岁；
③无内镜治疗禁忌症；
④经影像学证实无远处转移及周围侵犯；
⑤患者及家属均对本研究知情同意，签署知情同意书。

Inclusion criteria

①Early carcinoma of the esophagus considered by pathological examination or endoscopic fine examination, with lesions occupying ≥ 75% of the periaqueductal mucosa; post-ESD traumatic mucosal defect ≥ 75%;
②Age 18 to 80 years ol;
③No contraindications of endoscopic therapy;
④Absence of distant metastases and peripheral invasion as confirmed by imaging;
⑤Patients and family members gave informed consent to this study by signing the informed consent form.

排除标准：

①合并其他恶性肿瘤者或其他食管疾病者；
②合并器官功能障碍者；
③有控制不佳的糖尿病、股骨头坏死、消化性溃疡和肺结核；
④治疗依从性差，不能完成随访者；
⑤妊娠或哺乳期妇女；
⑥存在内镜手术禁忌者。如既往长期口服全身皮质醇激素及抗凝抗血小板药物。

Exclusion criteria：

①Patients with other malignant tumors or other esophageal diseases;
②Patients with organ dysfunction;
③Poorly controlled diabetes, necrosis of the femoral head, peptic ulcer and tuberculosis;
④Poor treatment compliance, unable to complete follow-up;
⑤Pregnant or lactating women;
⑥Patients with contraindications to endoscopic surgery. Such as long-term oral systemic cortisol hormone and anticoagulant antiplatelet drugs.
