研究目的：

（1）研究在下肢长骨骨折闭合复位髓内钉中髓腔内注射氨甲环酸是否能降低围手术期的隐性失血量及降低输血率，提高术后血红蛋白水平，并评价其应用的有效性。 （2）研究下肢长骨骨折闭合复位髓内钉中髓腔内注射氨甲环酸能否降低术后并发症发生率和死亡率，能否增加围手术期深静脉血栓的发生率，并评价其应用的安全性。  

Objectives of Study：

(1) To study whether intramedullary injection of tranexamic acid in closed reduction intramedullary nailing of long bone fractures of the lower extremities can reduce the amount of occult blood loss and reduce the rate of blood transfusion in the perioperative period and improve the postoperative hemoglobin level, and to evaluate the effectiveness of its application. (2) To study whether intramedullary injection of tranexamic acid in closed reduction intramedullary nailing of long bone fractures of lower limbs can reduce the incidence of postoperative complications and mortality, increase the incidence of perioperative deep vein thrombosis, and evaluate the safety of its application.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

因单侧下肢骨干骨折（胫骨、股骨）拟行闭合复位髓内钉治疗的患者。

Inclusion criteria

Patients with unilateral lower extremity diaphyseal fractures (tibia, femur) to be treated with closed reduction intramedullary nailing.

排除标准：

a.多发性创伤或骨折；
b.开放性、病理性骨折或假体周围骨折；
c.明确对氨甲环酸过敏或有使用禁忌；
d.血小板及凝血功能异常；
e.术前存在下肢深静脉血栓患者（肌间静脉血栓和深静脉血栓）。

Exclusion criteria：

a. Multiple trauma or fractures.
b. Open, pathological fractures or periprosthetic fractures.
c. Clear hypersensitivity to tranexamic acid or contraindication to its use.
d. Abnormal platelet and coagulation function.
e. Patients with preoperative lower extremity deep vein thrombosis (intermuscular vein thrombosis and deep vein thrombosis).
