研究目的：

食管气管瘘患者表现为进食呛咳，反复呼吸道感染和肺部感染，患者常因吸入性肺炎 、窒息 、营养不良和败血症等导致死亡。治疗方法有保守治疗或手术治疗，覆膜金属支架治疗食管气管瘘现已成为临床治疗食管气管瘘有效的手段，但对于巨大食管气管瘘的患者，普通的覆膜食管支架可直接通过瘘口压迫气管，导致气管狭窄甚至闭塞，患者表现为刺激性咳嗽，甚至呼吸衰竭，本研究通过改良的覆膜食管支架减轻对瘘口周围食管壁及气管的压力，避免气管受压，同时可保证食管腔通畅，该方法可是一种微创、并发症少、费用相对低廉的治疗方法，值得临床应用。  

Objectives of Study：

Patients with esophagotracheal fistula are characterized by food choking cough, recurrent respiratory tract infection and lung infection. Patients often die from aspiration pneumonia, asphyxia, malnutrition and septicemia. Treatment methods include conservative treatment or surgical treatment. Covered metal stent has become an effective method for clinical treatment of esophagotracheal fistula. However, for patients with huge esophagotracheal fistula, common covered metal stent can directly compress trachea through fistula opening, resulting in stenosis or occlusion of trachea. Patients show irritant cough and even respiratory failure. In this study, the improved covered metal stent reduces the pressure on esophageal wall and trachea around fistula opening, avoids tracheal compression, and ensures patency of esophageal lumen. This method is a minimally invasive, less complicated and relatively low-cost treatment method, which is worthy of clinical application.

纳入标准：

术前经内镜及X线钡剂造影明确病变部位及瘘口直径，同时瘘口直径大于2.0cm，麻醉风险高或手术困难者，术前与患者及家属充分沟通手术方式，术中、术后存在的风险及并发症、手术获益，经同意签字后开展此手术。

Inclusion criteria

Before operation, endoscopic and X-ray barium radiography shall be used to determine the lesion site and fistula diameter. If the fistula diameter is larger than 2.0 cm, the anesthesia risk is high or the operation is difficult, the operation method shall be fully communicated with the patient and his family members before operation, the risks, complications and benefits during and after operation, and the operation shall be carried out after approval and signature.

排除标准：

有外科手术适应症且患者有意愿者。
