研究目的：

本研究拟开展单中心随机对照试验研究，拟收集Ⅲ型食管闭锁患儿40例，通过中心分层动态随机分配方案将患者随机分配至单纯结扎及缝扎组中，通过一系列主要和次要观察指标评估患儿术后并发症的发生情况，探讨更好的手术方式。  

Objectives of Study：

This study plans to conduct a single-center randomized controlled trial. It is planned to collect 40 patients with Gross type C esophageal atresia , and randomly assign the patients to transfixing suture and ligation group through the center stratified dynamic random allocation scheme .The incidence of postoperative complications in children was assessed through a series of primary and secondary outcome measures to explore better surgical approaches.

药物成份或治疗方案详述：

将符合纳入标准的患者通过中心分层动态随机分配方案将患者随机分配至缝扎及单纯结扎组中：A 组：：胸腔镜手术下III型食管闭锁 远端气管食管瘘采用单纯结扎后离断的方式处理，余采用标准化治疗方案；B 组：胸腔镜手术下 III型食管闭锁远端气管食管瘘采用缝合结扎后离断的方式处理，余采用标准化治疗方案。 

Description for medicine or protocol of treatment in detail：

The patients who met the inclusion criteria were randomly assigned to the transfixing suture and ligation groups through the center stratified dynamic random allocation scheme: Group A: Gross type C esophageal atresia under thoracoscopic surgery and distal tracheoesophageal fistula were separated by ligation .Group B:Gross type C esophageal atresia and distal tracheoesophageal fistula under thoracoscopic surgery were treated by suture ligation and then severed, and the rest were treated with standardized treatment. 

纳入标准：

2021年11月至2023年11月期间在首都医科大学附属北京儿童医院就诊，经临床评估和影像学检查初步诊断为Ⅲ型食管闭锁；入院年龄≤7 天，出生体重≥2kg 并且≤4kg；家属签字同意加入本研究；签署知情同意。

Inclusion criteria

From November 2021 to November 2023, the patient was treated at Beijing Children's Hospital Affiliated to Capital Medical University, and was diagnosed with Gross type C esophageal atresia after clinical evaluation and imaging examination,admission age ≤7 days, birth weight ≥2kg and ≤4kg; family members signed to agree to join this study and signed informed consent.

排除标准：

合并重症肺炎，术前需要有创呼吸支持；合并严重心脏畸形；合并喉裂；合并其他危及生命的重大畸形的；纤维支气管镜检查发现食管近端与气管连接形成气管食管瘘；不同意胸腔镜手术；对治疗或临床研究依从性差者；拒绝参加本研究者

Exclusion criteria：

Those who was complicated with severe pneumonia requiring invasive respiratory support before surgery; those who complicated with severe cardiac malformation,laryngeal fissure and other major life-threatening malformations; those who underwent bronchoscopy and found tracheoesophageal fistula formed by the connection of the proximal end of the esophagus to the trachea; disagree with thoracoscopy Operation and those who disagreed with thoracoscopic surgery.those who had poor compliance with treatment or clinical research; those who refused to participate in this researcher.
