研究目的：

比较经鼻间歇正压通气（NIPPV）与经鼻持续正压通气（NCPAP）作为早期呼吸支持模式治疗胎龄<32 周早产儿呼吸窘迫综合征，减少有创呼吸支持的需求。  

Objectives of Study：

To compare nasal intermittent positive pressure ventilation (NIPPV) and nasal continuous positive pressure ventilation (NCPAP) as early respiratory support modes in the treatment of preterm infants with gestational age <32 weeks with respiratory distress syndrome and reduce the need for invasive respiratory support.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.出生胎龄<32 周，日龄≤6h（胎龄评估根据孕早期超声及孕妇末次月经共同决定）；
2.6h 内出现进行性呼吸窘迫相关表现，存在下列表现中至少 2 项：气促（呼吸次数>60次/分）、鼻翼扇动、呻吟、三凹征（胸骨上窝、肋间隙、剑突下）阳性、紫绀且生后6小时内胸片或肺部超声检查符合NRDS 表现；
3. 自主呼吸活跃；
4. 特殊情况说明：
（1）产房需气管插管复苏但到达病房后自主呼吸活跃者纳入研究；
（2）生后自主呼吸弱给予有创通气，但生后6小时内自主呼吸活跃临床医生认为可符合无创通气指征着纳入研究。

Inclusion criteria

1. The gestational age at birth is less than 32 weeks, and the daily age is less than or equal to 6 hours (the assessment of gestational age is based on the first trimester ultrasound and the last menstrual period of the pregnant woman);
2. Progressive respiratory distress-related manifestations within 6 hours, with at least 2 of the following manifestations: shortness of breath (breath rate > 60 breaths/min), alar flap, moaning, three concave sign (suprasternal fossa, intercostal space, subxiphoid) positive, cyanosis and consistent with NRDS findings on chest X-ray or lung ultrasonography within 6 hours after birth;
3. Active spontaneous breathing;
4. Description of special circumstances:
(1) Those who needed tracheal intubation resuscitation in the delivery room but were active spontaneously breathing after arriving in the ward were included in the study;
(2) Invasive ventilation was given to patients with weak spontaneous breathing after birth, but active spontaneous breathing within 6 hours after birth. Clinicians considered that they could meet the indications of non-invasive ventilation and were included in the study.

排除标准：

1.入院 6h 内死亡或因床位不足转院；
2.合并有青紫型心脏病、膈疝、先天性重大畸形或染色体异常、神经肌肉疾病。

Exclusion criteria：

1. Death within 6 hours of admission or transfer due to insufficient beds;
2. Combined with cyanotic heart disease, diaphragmatic hernia, major congenital malformation or chromosomal abnormality, neuromuscular disease.
