研究目的：

评估肺癌根治术中保留肺迷走神经彻底清扫纵隔淋巴结的安全可行性以及在术后快速康复的优越性。  

Objectives of Study：

To evaluate the safety and feasibility of preserving the vagus nerve in complete dissection of mediastinal lymph nodes during thoracoscopic radical resection of lung cancer and the superiority of enhanced recovery after surgery.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 年龄在 18-70 周岁之间；
2) 术前影像学诊断二级支气管以下的右侧支气管肺癌，需行单一肺叶切除手术患者；
3) 影像学显示肿瘤最大直径 ≤ 5cm( 2b)且未提示远处转移(M=0)；
4) 无冠心病、糖尿病基础疾病；
5) 麻醉评分(美国麻醉医师协会标准)≤ 2级；
6) 心肺功能(EF>50%， FEV1%>50%预计值，FEV1/FVC＞70%及其他重要脏器功能基本正常，静息不吸氧状态下血气分析 PO2≥75mmHg，PCO2<45mmHg，可耐受肺叶切除手术的；
7) 非吸烟患者。

Inclusion criteria

1) Aged 18 to 75 years;
2) Preoperative imaging diagnosis of right bronchogenic carcinoma below the secondary bronchus, requiring single lobectomy
3) Imaging showed that the maximum diameter of the tumor was no more than 5cm(2b) and no distant metastasis was discovered (M=0);
4) No coronary heart disease or diabetes;
5) Anesthesia score (American association of anesthesiologists standard) ≤2;
6) Cardiopulmonary function is tolerable to lobectomy(EF>50%, FEV1% >50% predicted value, FEV1/FVC > 70% and other important organ functions are basically normal, blood gas analysis PO2≥75mmHg, PCO2<45mmHg);
7) Non-smoking patients.

排除标准：

1) 术前有明确的咳嗽病史；
2) 明确合并引起慢性咳嗽的因素：鼻后滴流综合征、胃食管反流性疾病、口服血管紧张素转化酶抑制剂药物、肺结核、慢性支气管炎、COPD、支气管哮喘；
3) 合并可能造成术后引流量增加的病史、手术史或疾病：如明确结核病史、右侧胸腔有手术史；术前胸水 B 超示中量或中量以上胸腔积液;合并肺部感染等;或合并其它研究者认为可能造成术后漏气的病史或临床表现，如伴有弥漫性肺气肿等；
4) 影像学见存在异位腔静脉等解剖异常可能造成迷走神经辨认困难的;肺门、纵隔淋巴结有钙化或淋巴结融合表现的；
5) 合并其他绝对手术禁忌症或患有其他类型恶性肿瘤、月经期、妊娠等其他研究者认为不适合入组的其它情况；
6) 失访或其他原因不能明确是否发生持续咳嗽者。

Exclusion criteria：

1) History of cough before surgery;
2) Factors that may cause chronic cough: postnasal drip syndrome, gastroesophageal reflux disease, oral angiotensin converting enzyme inhibitor drugs, tuberculosis, chronic bronchitis, COPD, bronchial asthma;
3) Medical history, surgical history or disease that may lead to increased postoperative drainage: such as a history of tuberculosis and a surgical history in the right thoracic cavity;preoperative pleural effusion showed medium or above amount by B ultrasound;pulmonary infection and so on;or combined with other medical history or clinical manifestations that may cause postoperative airleakage, such as accompanied by diffuse emphysema;
4) Imaging findings that anatomic abnormalities such as ectopic vena cava may cause difficulty in identifying vagus nerve;hilar and mediastinal lymph nodes with calcification or lymph node fusion;
5) Patients with other absolute contraindications for surgery or other types of malignant tumors, menstrual period, pregnancy and other conditions considered unsuitable for enrollment by researchers;
6) Lost of follow-up or other reasons can not be clear whether there is persistent cough.
