研究目的：

探讨以家庭为基础的心脏康复治疗对降低老年冠心病患者衰弱的效果，以探索出能降低老年冠心病患者不良事件发生率、改善患者心功能状况、心理状态和生命质量的有效方法。  

Objectives of Study：

To explore the effect of family-based cardiac rehabilitation therapy on reducing frailty in elderly patients with coronary heart disease, in order to explore effective methods that can reduce the incidence of adverse events in elderly patients with coronary heart disease and improve their cardiac function, psychological state and quality of life.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄≥65 岁；
2.CHS 衰弱量表得分≥1 分；
3.符合卫生部发布的卫生行业标准中冠心病的诊断标准，满足其中一项即可：
（1）典型临床症状、心电图及心肌标志物监测符合国际心脏学会和协会（ISFS）及世界卫生组织（WTO）发布的诊断和治疗指南；
（2）选择性冠状动脉造影有 1 支或 1 支以上主要冠状动脉血管狭窄 ≥50%；
（3）典型心绞痛症状或既往有明确陈旧性心肌梗死病史；
4.处于疾病稳定期（病情或症状在一定量的时间内不会轻易的变化）；
5.无语言交流障碍；
6.知情且自愿参加本次研究者，患者依从性好，愿意配合出院后的随访工作及康复治疗方案者；
7.患者有智能手机，并能正确使用。

Inclusion criteria

1. Aged >= 65 years;
2. CHS frailty scale score >= 1 point;
3. Meet the diagnostic criteria for coronary heart disease in the health industry standards issued by the Ministry of Health, and meet one of the following:
(1) Typical clinical symptoms, ECG and myocardial marker monitoring comply with the diagnostic and treatment guidelines issued by the International Society and Society of Cardiology (ISFS) and the World Health Organization (WTO);
(2) Selective coronary angiography with 1 or more major coronary artery stenosis >=50%;
(3) Typical symptoms of angina pectoris or a clear history of old myocardial infarction;
4. In the stable disease stage (the condition or symptoms will not change easily within a certain period of time);
5. No language barriers;
6. Those who are informed and willing to participate in this researcher, the patients have good compliance, and are willing to cooperate with the follow-up work and rehabilitation treatment plan after discharge;
7. The patient has a smartphone and can use it properly.

排除标准：

1.中高危患者，如合并大面积心梗、恶性心律失常、心源性休克等；
2.存在周围血管疾病、肾功能不全、恶性肿瘤、贫血、严重肺部疾病等；
3.有影响运动的骨关节疾病；
4.参加过心脏康复功能锻炼者。

Exclusion criteria：

1. Intermediate and high-risk patients, such as large-area myocardial infarction, malignant arrhythmia, cardiogenic shock, etc.;
2. Peripheral vascular disease, renal insufficiency, malignant tumor, anemia, severe lung disease, etc.;
3. Bone and joint diseases that affect movement;
4. Those who have participated in cardiac rehabilitation exercise.
