研究目的：

综合全身各系统，寻找影响脱机的因素，制定困难脱机早期诊断预测体系，以标准化的诊断预测体系，代替临床医生的主观判断，以期降低拔管失败率，并避免不必要机械通气时间的延长，进而改善患者预后。  

Objectives of Study：

In order to reduce the failure rate of extubation and avoid the prolongation of unnecessary mechanical ventilation time and improve the prognosis of patients, we should synthesize all the systems of the whole body, find out the factors affecting the off-line, formulate the early diagnosis and prediction system for difficult off-line, and replace the subjective judgment of clinicians with the standardized diagnosis and prediction system.

药物成份或治疗方案详述：

该研究设计为一项包括多中心回顾性观察性研究与多中心前瞻性观察性研究。将从全中国3个省市（北京、河北、天津）中筛选6家具有代表性的ICU的医院参与研究。为研究影响脱机结果的因素，并制订预测脱机结果的早期诊断预测体系，在6家医院中选择住ICU时间大于24小时的机械通气患者，根据2017年WIND分类标准，将患者分为4组，进而寻找各组间有统计学差异的因素，进而运用logistic回归分析寻找影响脱机结果的因素。 

Description for medicine or protocol of treatment in detail：

The study was designed as a multi-center retrospective observational study and a multi-center prospective observational study. Six representative ICU hospitals will be selected from three provinces in China (Beijing, Hebei and Tianjin). In order to study the factors affecting the results of off-line, and to establish an early diagnosis and prediction system for predicting the results of off-line, mechanical ventilation patients who stayed in ICU for more than 24 hours were selected from six hospitals. According to the WIND classification criteria in 2017, patients were divided into four groups, and then the factors with statistical differences among groups were found. Then the factors affecting the results of off-line were analyzed by logistic regression. 

纳入标准：

1.ICU收入的有创机械通气患者
2.年龄≥18岁
3.提供知情同意书表达愿意参与研究（仅适用于前瞻性研究部分）。

Inclusion criteria

1. Invasive mechanical ventilation patients in ICU;
2. Aged at least 18 years;
3. Provide informed consent to express willingness to participate in research (only for prospective research).

排除标准：

1.入ICU时已行气管切开患者
2.第1次脱机尝试前出院的患者
3.住ICU时间少于24小时的患者
4.经鼻气管插管患者

Exclusion criteria：

1. Patients who had undergone tracheotomy at the time of admission to ICU;
2. Patients discharged before the first SBT attempt;
3. Patients who live in ICU for less than 24 hours;
4. Patients with nasotracheal intubation.
