研究目的：

本项目将通过比较瑞马唑仑与丙泊酚麻醉，并与普通胃肠镜检查对照，观察胃肠镜检查患者胃肠动力和血浆胃肠激素水平的变化，探讨瑞马唑仑对胃肠运动的影响，研究结果将拓展舒适化医疗的内容和范畴，提高无痛胃肠镜患者的恢复质量。  

Objectives of Study：

This project will observe the changes of gastrointestinal motility and plasma gastrointestinal hormone levels in patients undergoing gastrointestinal endoscopy by comparing the anesthesia of remimazolam and propofol, and compare it with ordinary gastrointestinal endoscopy, so as to explore the impact of remimazolam on gastrointestinal motility. The research results will expand the content and scope of comfort medicine and improve the recovery quality of patients undergoing painless gastrointestinal endoscopy.

药物成份或治疗方案详述：

根据患者意愿选择普通胃肠镜检查或者无痛胃肠镜检查。无痛胃肠镜检查患者，按随机数字表法被随机分入瑞马唑仑组或丙泊酚组，接受相应药物镇静麻醉。建立药物使用登记表格，并认真填写。所有患者常规胃肠道准备，检查前均口服局麻药达克罗宁。瑞马唑仑组（R组）静脉依次注射舒诺扬0.2mg+瑞马唑仑0.2~0.25mg/Kg，丙泊酚组（P组）静脉依次注射诺扬0.2mg+丙泊酚1.5~2mg/kg。待所有患者OAA/S评分为0时开始行胃肠镜检查，3~4min钟后或发生体动时两组分别追加瑞马唑仑2.5mg/次或丙泊酚0.5mg/kg。检查过程中维持血压在基础值±20%以内，心率±30%以内，SPO2≥90%。 离开PACU标准： 检查结束后将患者送入PACU苏醒，待PADSS评分≥9分且患者无眩晕、站立不稳，无气道阻塞和呼吸困难等症状时，准许患者在陪同下离开PACU。 

Description for medicine or protocol of treatment in detail：

Choose ordinary gastroenteroscopy or painless gastroenteroscopy according to the wishes of patients. Patients undergoing painless gastroenteroscopy were randomly divided into rimazole group or propofol group according to the random number table method, and received corresponding drugs for sedation and anesthesia. Establish drug use registration form and fill it in carefully. All patients received routine gastrointestinal preparation and oral local anesthetic dacronin before examination. Group R received intravenous injection of 0.2mg sunoyan + 0.2 ~ 0.25mg/kg rimazolin, and group P received intravenous injection of 0.2mg sunoyan + 1.5 ~ 2mg/kg propofol. Gastroenteroscopy was started when OAA / s score of all patients was 0. Rimazolom 2.5mg/time or propofol 0.5mg/kg were added to the two groups after 3 ~ 4min or when body movement occurred. During the examination, maintain blood pressure within ± 20% of the basic value, heart rate within ± 30%, SpO2 ≥ 90%. Departure from PACU standard: After the examination, send the patient to PACU to wake up. When the padss score is ≥ 9 and the patient has no symptoms such as vertigo, unstable standing, airway obstruction and dyspnea, the patient is allowed to leave PACU accompanied. 

纳入标准：

（1）单次麻醉下行无痛胃镜及肠镜检查患者；
（2）ASA I-II级；
（3）年龄18-65岁，性别不限；
（4）体重指数（BMI）：28kg/m2≥BMI≥18kg/m2；

Inclusion criteria

(1) Painless gastroscopy and enteroscopy under single anesthesia;
(2) ASA class I-II;
(3) Age 18-65 years old, regardless of gender;
(4) Body mass index (BMI): 28kg/m2≥BMI≥18kg/m2

排除标准：

（1）依从性差，不能配合者；
（2）妊娠或哺乳期女性；
（3）行内镜下手术治疗患者（息肉摘除，ESD等手术）；
（4）近期服用影响胃肠动力药物（吗丁啉、莫沙必利、M1受体激动/抑制剂等）或镇静药物；
（5）胃肠镜检查时间过长（≥20min）；
（6）对阿片类、苯二氮卓类或丙泊酚药物过敏。

Exclusion criteria：

（1）Poor compliance and inability to cooperate;
（2）Pregnant or lactating women;
（3）Endoscopic surgery (polypectomy, ESD, etc.);
（4）Recent use of drugs affecting gastrointestinal motility (morpholine, mosapride, M1 receptor agonists / inhibitors) or sedative drugs;
（5）The time of gastroenteroscopy is too long (≥ 20min);
（6）Allergic to opioids, benzodiazepines or propofol
