探讨个体化吞咽治疗对喂养障碍早产儿经口喂养能力的影响  

Objectives of Study：

To explore the effect of individualized swallowing therapy on oral feeding ability of preterm infants with feeding disorder.

药物成份或治疗方案详述：

通过随机对照的方法，控制混杂因素的干扰，对不同胎龄组早产儿进行疾病严重程度、新生儿口腔运动功能、非营养性吸吮能力、早产儿准备经口喂养能力等系统评估，分析各评估指标及日均增加奶量(mL)、日均增加体重(g)、留置胃管的时间、全经口喝奶的时间(从开始治疗到全面经口喝奶的过渡时间)、总住院时间的差异，以评价常规吞咽治疗及个体化吞咽治疗方法对不同胎龄组早产儿经口喂养能力的影响。 

Description for medicine or protocol of treatment in detail：

Through the method of random control and controlling the interference of confounding factors, the disease severity, neonatal oral motor function, non nutritional sucking ability, and the ability of preterm infants to prepare oral feeding were systematically evaluated for preterm infants in different gestational age groups. The evaluation indexes and the average daily added milk volume (ML), average daily added weight (g), the time of indwelling gastric tube were analyzed To evaluate the effects of routine swallowing therapy and individualized swallowing therapy on the oral feeding ability of preterm infants of different gestational age groups. 

纳入标准：

(1)符合《实用新生儿学》早产儿诊断标准，矫正胎龄28-34周：
(2)出生体重800g～3000g；
(3)生命体征平稳，治疗时不需要呼吸机辅助呼吸。
(4)未经口进食，有喂养障碍，需经留置胃管进食。

Inclusion criteria

(1) It meets the diagnostic criteria of premature infants in practical neonatology, and the corrected gestational age is 28-34 weeks:

(2) Birth weight: 800g ~ 3000g;

(3) Vital signs are stable, and there is no need for ventilator to assist breathing during treatment.

(4) If you have feeding disorder without oral feeding, you need to eat through indwelling gastric tube.

排除标准：

(1)生命体征尚不平稳，需呼吸机辅助呼吸。
(2)有先天性代谢性疾病或先天发育畸形，如消化道畸形、口腔畸形（唇裂、腭裂）等。
(3)合并严重并发症，如严重的支气管肺发育不良、大于Ⅲ级的颅内出血、坏死性小肠结肠炎、存在明显血流动力学异常的先天性心脏病等。

Exclusion criteria：

(1) Vital signs are not stable and need to be assisted by ventilator.

(2) There are congenital metabolic diseases or congenital developmental malformations, such as gastrointestinal malformations, oral malformations (cleft lip, cleft palate), etc.

(3) Complicated with serious complications, such as severe bronchopulmonary dysplasia, intracranial hemorrhage greater than grade III, necrotizing enterocolitis, congenital heart disease with obvious hemodynamic abnormalities, etc.
