研究目的：

关于珂立苏治疗新生儿急性呼吸窘迫综合征的研究较少，样本量小，关于其在真实世界应用中的有效性、安全性的研究更少，且其在真实世界应用中的最佳剂量、使用时间等尚不明确。故本研究拟前瞻性收集全国24家三级医院2022年3月至2024年3月关于ARDS的诊疗数据，(1)评价珂立苏在真实诊疗环境中治疗新生儿急性呼吸窘迫综合征的有效性和安全性；(2)探索不同剂量珂立苏的疗效以及影响其疗效的因素；(3)探索珂立苏治疗新生儿急性呼吸窘迫综合征的最佳剂量。  

Objectives of Study：

There are few studies on the treatment of neonatal acute respiratory distress syndrome, small sample size, fewer studies on its effectiveness and safety in real-world applications, and its optimal dose and duration of use in real-world applications are unclear.Therefore, this study intends to prospectively collect data on ARDS in 24 tertiary hospitals from March 2022 to March 2024, (1) evaluate the effectiveness and safety of Colisu in real diagnosis and treatment environment; (2) explore the efficacy of different doses of Colisu and the factors affecting its efficacy; and (3) explore the optimal dose of Colisu in the treatment of neonatal ARDS.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

选取全国三级医院新生儿病房，并可应用机械通气治疗的24家综合医院、妇幼保健院及儿童医院组成“全国新生儿 ARDS研究协作组”，选择 2022 年 03月至 2024 年 03月各协作医院收治的符合选入排除标准的患者为前瞻性研究的对象，收集其相应临床数据资料并填写相应调查表。
选入/排除标准
符合“蒙特勒标准”的新生儿，其诊断标准如下：
（1）明确或可疑诱因（窒息、呛奶、胎粪吸入和感染等）后出现的急性发作（1 周内）；
（2） 排除新生儿呼吸窘迫综合征（RDS）、新生儿暂时性呼吸增快（TTN）、肺表面活性物质（PS）相关遗传性缺陷或先天性畸形等引起的呼吸困难；
（3）肺部影像学表现为双侧弥漫性不规则的透光度下降、渗出或白肺，这些改变不能用其他原因解释（如局部积液、肺不张、RDS、TTN 或先天性畸形等）；
（4）出现先天性心脏病无法解释的肺水肿，心脏超声可用于证实肺水肿原因；
（5）出现氧合障碍，以氧合指数（oxygenation index, OI）高低评估氧合障碍程度。
根据 OI 高低将新生儿 ARDS 严重程度分为：轻度（4≤OI < 8）、中度（8≤OI < 16）和重度（OI≥16）。（；由于平均气道压对有创呼吸支持治疗的新生儿易获得，对于无创呼吸支持的新生儿，精确的平均气道压较难获得，此种情况下可通过在颌部施加压力并使用适当大小的鼻塞来得到其呼吸道泄漏最小化时的平均气道压，以此来计算OI。）

Inclusion criteria

Select the national tertiary hospital neonatal ward, and can apply mechanical ventilation treatment of 24 general hospitals, maternal and child health care and children's hospital "national neonatal ARDS research collaboration group", choose March 2022 to 2024 patients meet the exclusion criteria for prospective study, collect the corresponding clinical data and fill in the corresponding questionnaire.
Selected entry / exclusion criteria
Neonborns who meet the "Montreux Standards" have the following diagnostic criteria:
(1) Acute onset (within 1 week) after clear or suspicious causes (asphyxia, milk choking, meconium inhalation and infection, etc.);
(2) Excluding respiratory difficulties caused by neonatal respiratory distress syndrome (RDS), neonatal temporary respiratory acceleration (TTN), pulmonary surfactant (PS) -related genetic defects or congenital malformations;
(3) Pulmonary imaging shows bilateral diffuse irregular decline in transmittance, exudation or white lungs, which changes cannot be explained by other reasons (such as local effusion, atelectasis, RDS, TTN or congenital malformation, etc.);
(4) With congenital heart disease, cardiac ultrasound can be used to confirm the cause of pulmonary edema;
(5) If, the degree of oxygenation disorder is evaluated by oxygenation index (oxygenation index, OI).
According to the OI level, neonatal ARDS severity was classified into mild (4 OI <8), moderate (8 OI <16) and severe (OI 16).(; Because the mean airway pressure is easily accessible to neonates with invasive respiratory support therapy, the precise mean airway pressure is more difficult to obtain for newborns with noninvasive respiratory support, and the OI can be calculated by applying pressure on the jaw and using the appropriate sized nasal obstruction.）

排除标准：

1.排除新生儿呼吸窘迫综合征（RDS）、新生儿暂时性呼吸增快（TTN）、肺表面活性物质（PS）相关遗传性缺陷或先天性畸形等引起的呼吸困难；
2.肺部影像学表现为双侧弥漫性不规则的透光度下降、渗出或白肺，这些改变可以用局部积液、肺不张、RDS、TTN 或先天性畸形等解释的；

Exclusion criteria：

1. Excluding respiratory difficulties caused by neonatal respiratory distress syndrome (RDS), neonatal temporary respiratory acceleration (TTN), pulmonary surfactant (PS) -related genetic defects or congenital malformations;
2. Pulmonary imaging shows bilateral diffuse irregular decline in light transmittance, exudation or white lung, and these changes can be explained by local effusion, atelectasis, RDS, TTN or congenital malformations;
