研究目的:

心肌缺血再灌注损伤(Myocardial?ischemic?reperfusion,?MIR)常见于围术期,严重危害患者生命,但发病机制尚未阐明。课题组前期发现位于缺血心肌局部及背根神经节(Dorsal?root?ganglia,?DRG)、脊髓神经元上的TRPV1通道参与MIR损伤,且MIR后心肌及血清中TGF-β1含量上调。而在炎症性损伤、病理性疼痛所致神经异常放电中,TGF-β1对神经元TRPV1通道具有敏化作用。由此,我们推测TGF-β1敏化TRPV1参与MIR的形成。本项目将进一步探讨具体敏化通路,以期为围术期患者管理及心肌保护提供新的理论支持。  

Objectives of Study:

Myocardial ischemia-reperfusion injury (MIR) is widespread during cardiac and non-cardiac surgery, increasing the rate of perioperative mortality and morbidity. A variety of inflammatory mediators released during ischemia and reperfusion, stimulate or sensitize cardiac afferent nerves and transmit ischemic nociceptive signals to the thoracic spinal cord through dorsal root ganglia (DRG). This results in cardiac excitatory cardiovascular reflex responses and breaking the balance of myocardial oxygen supply and demand. The previous research found that TRPV1 channel located in the ischemic myocardium, DRG and spinal cord neurons participate in MIR injury; the level of TGF-β1 in the myocardium and serum is up-regulated after MIR. The sensitization of TRPV1 induced by TGF-β1 is the mechanism of inflammatory injury and pathological pain, and whether TGF-β1-induced TRPV1 sensitization is involved in the MIR is unknown. we observe the mechanism of TGF-β1 sensitizing TRPV1 in MIR, which will provide new theoretical support for perioperative patient management and myocardial protection.

纳入标准:

2023年1月1日至2023年6月30日,在我院就诊,并拟施体外循环下双瓣置换的患者。

Inclusion criteria

From January 1, 2023 to June 30, 2023, patients were admitted to our hospital and planned to receive double valve replacement under cardiopulmonary bypass.

排除标准:

术前凝血功能异常、严重的心衰、纽约心功能分级VI级,存在癌症、内分泌系统或免疫系统疾病。

Exclusion criteria:

Preoperative coagulation abnormalities, severe heart failure, New York Heart Function Class VI, presence of cancer, endocrine system or immune system disease.

本研究中观察接受心脏瓣膜置换手术的患者围术期血样本中肌钙蛋白及TGFβ1的变化,以此探究心肌经历缺血再灌注损伤过程中TGFβ1及肌钙蛋白在其中的作用。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

In this study, the changes of serum troponin and TGFβ1 in perioperative blood samples of patients undergoing cardiac valve replacement surgery were observed to investigate the role of TGFβ1 and troponin in myocardial ischemia-reperfusion injury.
