研究目的:

乳腺癌内分泌治疗后引起的骨代谢异常是一种常见的并发症,至今尚无一种调节骨微环境稳态的有效药物,中医临床证实该阶段患者发病多为肝肾两虚,补益肝肾类方药治疗该阶段骨质疏松疗效确切,且不影响内分泌治疗效果,但其相关治则治法实验研究尚未取得突破性进展。我们临床应用发现,以补益肝肾法为指导的龟鹿三黄汤可改善骨代谢异常状态,但其作用机理尚不明确,已知三黄汤可通过影响乳腺癌患者肿瘤细胞所处内环境,降低氧化应激血清学相关指标,抗炎,改善血瘀指标,增强抗肿瘤疗效,所以临床使用龟鹿三黄汤针对肝肾两虚的乳腺癌内分泌治疗患者,我们假设其作用机制与调节患者体内的相关炎症介质IL-1、IL-6、TNF-α、IFN-γ及成骨细胞Ⅰ型胶原蛋白(COL1A1、COL1A2)、TGF-β1、TGF-β2 、骨代谢指标(OPG骨保护素,BGP骨钙素)相关,同时检测患者抗氧化应激情况、内分泌情况及其他相关安全指标,了解龟鹿三黄汤调节患者骨微环境代谢异常的相关作用机制及靶点,为开发防治肝肾两虚型乳腺癌内分泌治疗后引起骨质疏松类中药提供理论支撑。  

Objectives of Study:

Abnormal bone metabolism caused by endocrine therapy for breast cancer is a common complication. So far, there is no effective drug to regulate the homeostasis of bone microenvironment. Chinese medicine clinically proves that most patients at this stage suffer from deficiency of both liver and kidney. Recipes for tonifying liver and kidney have definite curative effect on osteoporosis at this stage, and do not affect the endocrine therapeutic effect. However, experimental research on the relevant therapeutic principles and methods has not been achieved. Breakthrough progress. Our clinical application has found that Guilu Sanhuang Decoction guided by tonifying liver and kidney can improve the abnormal state of bone metabolism, but its mechanism is still unclear. It is known that Sanhuang Decoction can affect the internal environment of cancer cells in breast cancer patients, reduce the serological indicators of oxidative stress, anti-inflammation, improve blood stasis indicators and enhance the anti-tumor effect. Therefore, Guilu Sanhuang Decoction is used clinically to treat breast cancer. In the endocrine therapy of breast cancer with deficiency of both liver and kidney, we hypothesize that the mechanism of action is related to the regulation of inflammatory mediators IL-1, IL-6, TNF-alpha, IFN-gamma and osteoblast type I collagen (COL1A1, COL1A2), TGF-beta 2, bone metabolism index (OPG osteoprotegerin, BGP osteocalcin) in patients with deficiency of both liver and kidney. To understand the mechanism and target of Guilu Sanhuang Decoction in regulating the abnormal metabolism of bone microenvironment in patients with breast cancer of deficiency of liver and kidney, and to provide theoretical support for the development of traditional Chinese medicine for preventing and treating osteoporosis caused by endocrine therapy for breast cancer of deficiency of liver and kidney.

药物成份或治疗方案详述:

 

Description for medicine or protocol of treatment in detail:

 

纳入标准:

a 经组织学确诊的乳腺癌,根据NCCN指南,有辅助内分泌治疗适应症;
b已完成手术、辅助化疗,和/或放疗;
c 年龄在30-80岁之间;
d有使用内分泌治疗的指征并愿意接受内分泌治疗,且正在服用内分泌治疗者;
e骨密度(双能X线骨密度仪测定腰椎和髋部):降低超过2.5个标准差为骨质疏松(T值≤-2.5)。
f符合中医证候肝肾亏虚证诊断标准者;
g预计生存期>6个月。

Inclusion criteria

1. Histology confirmed breast cancer, according to NCCN guidelines, has indications for adjuvant endocrine therapy;
2. Surgery, adjuvant chemotherapy and/or radiotherapy have been completed;
3. Aged between 30 and 80 years old;
4. Those who have indications of using endocrine therapy and are willing to accept endocrine therapy and are taking endocrine therapy;
5. Bone mineral density (lumbar spine and hip measured by dual-energy X-ray absorptiometry): Osteoporosis was reduced by more than 2.5 standard deviations (T value < 2.5).
6. Those who meet the diagnostic criteria of deficiency of liver and kidney in TCM syndromes;
7. The estimated survival time is more than 6 months.

排除标准:

a 晚期转移性乳腺癌;
b 依从性差不能配合者;
c 正在参加其他药物临床试验患者;
d伴有内分泌失调基础疾病而未控制者;

Exclusion criteria:

1. advanced metastatic breast cancer;
2. Those with poor compliance can not cooperate;
3. Patients who are participating in other drug clinical trials;
4. Those with underlying endocrine disorders without control.
