研究目的:

結合虛擬實境VR,以 Exergame 的方法進行中風患者復健治療,由虛擬的老師教授一致性的動 作,藉由調整動作的組數、施行的動作順序以及難易度建立適合中風患者的訓練模 式。此外,有文獻指出BDNF的表現量可被調控氧化壓力的HO-1或發炎反應影響,因 此,我們將檢驗患者進行訓練前後血漿中的8-OHdG、O2-、IL-6、TNF-α、BDNF 表 現量變化並進一步探討相關訊息傳遞路徑。本研究計劃將進行跨領域之研究,將分 子機轉結合職能治療之實證醫學進行探討  

Objectives of Study:

combine vr to deliver a standardized intervention, subject will follow the virtual coach with same movement, and every action will be recorded and scored by software. Our primary goal is to evaluate training dose and intensity in stroke patient by using Fugl-Meyer Assessment-lower extremity (FMA-LE), Maximum walking speed (MWS), Berg Balance Scale (BBS), Stroke Impact Scale (SIS), Modified Rankin Scale (MRS), Barthel Index (BI). Secondary, we measured safety and acceptance by record the adverse effect with occurrence frequency, and satisfaction rated by subjects.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

(1) 進入試驗前簽署同意書。
(2) 經腦部掃描(CT 或MRI) 證實為中風診斷。
(3) 發病日期至今為3 個月以上。
(4) 簡短智能測驗(Mini-Mental State
Examination) 分數大於20 分以上。
(5) 傅格-梅爾動作測驗(Fugl-Meyer
Assessment) 下肢運動總分大於10 分以上,相
當於Brunnstrom stage 5~6。
(6) 功能性步行功能分級量表(FAC) 3~5
級。
(7) 年齡為20~75 歲。

Inclusion criteria

patient age between 20 to 75 years, first onset of stroke over 3 months. Brunnstrom stage higher than 3, having a diagnosis with stroke and confirmed with Computed tomography or Magnetic Resonance Imaging scans, being able to understand instructions, Mini-Mental State Examination score higher than 18 and, no other neurological disorders as comorbid.

排除标准:

1) 在三個月內參與其他復健相關試驗或藥
物試驗。
(2) 有失用症(Apraxia) 的情況
(3) 有嚴重的視覺及視知覺損傷,如忽略
(neglect) 或視野範圍較小。
(4) 需要使用抗凝血劑Warfarin 或對
等劑量的維生素K 拮抗劑(例如:
phenprocoumon)。
(5) 個案因認知或人格因素無法與研究人
員合作、不願意簽署同意書。

Exclusion criteria:

participated in other rehabilitation related or clinical trials within 3 months of the experiment, having sensory apraxia, having severe impairments in vision or visual perception such as hemi-neglect, receiving warfarin or VitK antagonist treatment, having high risk of epilepsy, failure to cooperate with the researcher to execute vr activities and refusing to take the blood test
