比较神经内镜手术与非手术治疗中等量基底节区高血压脑出血的临床疗效与安全性  

Objectives of Study：

To compare effectiveness of endoscopic surgery with conservative treatment for moderate volume hematoma in spontaneous basal ganglia heamorrhage

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1、基底节区高血压脑出血，诊断符合1995年全国第四届脑血管病学术会议高血压脑出血诊断要点。
2、脑内血肿量为20~40ml，脑室内少量出血不计入出血量。
3、发病至入院时间在36小时内。
4、患者年龄在15岁至75岁之间。
5、GCS≥8分

Inclusion criteria

1. Spontaneous basal ganglia hemorrhage confirmed by computed tomography(CT).
2. The volume of the hematoma is between 20ml to 40ml. Small intraventricular hemorrhage is not counted.
3. Admission within 36 hours from the ictus.
4. Age between 15 to 75 years old.
5. Glasgow Coma Score(GCS) ≥ 8.

排除标准：

1、有脑疝征象者（一侧或双侧瞳孔散大、自主呼吸微弱或无自主呼吸、血压不能持续维持在正常范围）。
2、动脉瘤、血管畸形、肿瘤卒中、外伤或脑梗塞导致出血者。
3、合并脑室内大量出血（侧脑室内血肿面积大于其横切面1/2），三脑室、四脑室积血伴脑室扩大。
4、脑干、幕下出血、原发脑室内出血。
5、合并有血小板减少或凝血功能障碍者，或发病前40天中曾服用会影响凝血功能的药物者。
6. 发病前患有严重精神或躯体疾病，并导致明显残疾者。合并有严重感染、严重心、肺、肝、肾相关疾病者。
7、妊娠患者.

Exclusion criteria：

1. Any sign of brain herniation, including deep coma, unilateral or bilateral pupil dilation, abnormal posture, or unstable vital signs.
2. Hemorrhage caused by aneurysm, vascular malformations, tumor apoplexy, trauma, or cerebral infarction. Computed tomography angiography will be performed.
3. Large intraventricular hematomas which occupy half of the cross section area of the lateral ventricle on CT scans, or hydrocephalus caused by hematocele.
4. Primary intraventricular hemorrhage.
5. Dysfunction of blood coagulation, thrombocytopenia, or history of taking any kind of drugs which affect coagulation function in the past 40 days.
6. Before the ictus, the patient has severe physical or mental illness which causes significant disability, or the patient has other severe comorbidity.
7. The patient is pregnant.
