研究目的：

评价雷替曲塞腹腔灌注化疗预防结直肠癌术后腹腔种植转移的有效性和安全性  

Objectives of Study：

To evaluate the efficacy and safety of raltitrexed intraperitoneal lavage chemotherapy in the prevention of of postoperative abdominal metastasis of colorectal cancer

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 年龄为 18-70 岁（包括 18 岁和 70 岁），性别不限；
2) ECOG 评分≤1；
3) 经组织病理学证实的结直肠腺癌患者；
4) 临床分期为 T4NxM0 或 T3N+M0，肿瘤部位位于结肠或直肠上段；
5) 术前经影像学评估，可手术切除；
6) 治疗前1周内实验室检查结果满足以下条件：白细胞(WBC) ≥ 4.0×10^9/L；中性粒细胞计数(ANC) ≥ 2.0×10^9/L；血小板(PLT) ≥ 100×10^9/L；血红蛋白(Hb) ≥ 90 g/L；血清总胆红素(TB) ≤1.5×ULN；血清门冬氨酸氨基转移酶(AST)、血清丙氨酸氨基转移酶(ALT)、血清碱性磷酸酶(ALP) ≤ 2.5×ULN；血尿素氮(BUN)、血肌酐(Cr) ≤ 1.5×ULN；内生肌酐清除率(Cr) ≥ 60 ml/min (Cockcroft-Gault 公式)；
7) 患者自愿加入本研究，签署书面知情同意书，有良好的依从性并愿意配合随访。

Inclusion criteria

1) Aged 18-70 years old male or female;
2) ECOG <= 1;
3) Histopathological diagnosis of colorectal cancer;
4) TNM stage is T4NxM0 or T3N+M0, and the tumor site is located in the colon or upper rectum;
5) Preoperative imaging evaluation that possibility of operation excision;
6) The laboratory test within 1 week before treatment meet the following criteria:White blood cell (WBC)≥ 4.0×10^9/L; Absolute count of blood neutrophil (ANC) ≥2.0×10^9/L; Platelet (PLT) ≥100×10^9/L; Hemoglobin (HB) ≥90g/L; Serum total bilirubin (TB)≤1.5×ULN; aspartate aminotransferase (AST),alanine aminotransferase (ALT), serum alkaline phosphatase (ALP)≤ 2.5×ULN; Blood urea nitrogen (BUN),serum creatinine (Cr) ≤ 1.5×ULN; Endogenous creatinine clearance (Cr) ≥60 ml/min (calculated using the Cockcroft-Gault formula);
7) Patients volunteer to participate in the study and sign informed consent, have good compliance and are willing to cooperate with follow-up.

排除标准：

1) 合并严重并发症考虑不能耐受术后化疗者；
2) 既往或同时患有其他恶性肿瘤，但不包括已治愈的宫颈原位癌、皮肤基底细胞癌或皮肤鳞状细胞癌；
3) 伴有消化性溃疡、消化道动力性梗阻、消化道严重活动性出血、消化道穿孔等；
4) 已知对本方案药物组分或代谢产物有过敏史者；
5) 有免疫缺陷病史，包括 HIV 检测阳性，或患有其他获得性、先天性免疫缺陷疾病，或有器官移植史；
6) 曾患有任何心脏疾病，包括：(1)心绞痛；(2)需药物治疗的或有临床意义的心律失常；(3)心肌梗死；(4)心力衰竭；(5)任何被
研究者判断为不适于参加本试验的其他心脏疾病等；
7) 妊娠期、哺乳期女性患者，有生育能力且基线妊娠试验检测阳性的女性患者或在整个试验期间不愿意采取有效避孕措施的育龄女性患者；
8) 根据研究者的判断，有严重的危害患者安全、或影响患者完成研究的伴随疾病（包括但不限于高血压、严重的糖尿病、活动性感染、甲状腺疾病等）；
9) 既往有明确会影响研究知情或随访评价的神经或精神障碍史，例如重度抑郁、癫痫或痴呆等；
10) 在入组前 30 天内参与其他临床试验，并接受研究性药物和任何包含研究性药物的伴随治疗；
11) 研究者认为不宜参加本试验的其他情况。

Exclusion criteria：

1. Serious complications associated with inability to tolerate postoperative chemotherapy;
2. The patient has a history of other tumor,except for the treatment of carcinoma of the cervical carcinoma in situ and basal cell carcinoma or squamous carcinoma of the skin;
3. Accompanied by peptic ulcer, digestive tract motility obstruction, severe active gastrointestinal bleeding, digestive tract perforation;
4. Patients known to have a history of allergies to the drug components or metabolites of this regimen;
5. Patients have a history of immunodeficiency, including HIV positive, or other acquired, congenital immunodeficiency disease, or a history of organ transplantation;
6. Patients with any heart disease, including:
(1) angina;
(2) drug-treated or clinically significant arrhythmia;
(3) myocardial infarction;
(4) heart failure;
(5) Any other heart disease that the researcher judges to be unsuitable for this study;
7. Pregnancy, breast-feeding women; Female patients who have fertility and have a positive pregnancy test or are unwilling to take effective contraceptive measures throughout the trial;
8. According to the investigator's judgment, there are serious concomitant diseases (including but not limited to high blood pressure, severe diabetes, active infection, thyroid disease, etc.) that are harmful to the safety of the patient or affect the patient's completion of the study;
9. Previous history of neurological or psychiatric disorders that may affect research informed or follow-up evaluation, such as major depression, epilepsy or dementia;
10. Participate in other clinical trials within 30 days prior to enrollment and receive study medications and any concomitant treatments containing research medications;
11. The researchers did not believe that the case was suitable for the group.
