研究目的：

1、对比普通白光内镜和共聚焦内镜对慢性萎缩性胃炎的诊断价值； 2、评估普通白光内镜下诊断慢性萎缩性胃炎阳性，而病理活检结果为阴性的患者是否需要进一步的检查； 3、评估共聚焦内镜对慢性萎缩性胃炎的诊断准确率； 4、对普通白光内镜下诊断慢性萎缩性胃炎阳性，而病理活检结果为阴性的患者下一步的诊疗方案提供合理的建议和指导。  

Objectives of Study：

1.compare diagnostic value of ordinary endoscopy and confocal laser endomicroscopy in chronic atrophic gastritis; 2.assess whether patients with chronic atrophic gastritis,diagnosed positive by ordinary endoscopy and negative by biopsy,require further examination; 3.assess the accuracy of confocal laser endomicroscopy in the diagnosis of chronic atrophic gastritis; 4.provide reasonable advice and guidance for the next treatment plan for patients with chronic atrophic gastritis diagnosed positive by ordinary endoscopy and negative by biopsy.

药物成份或治疗方案详述：

1、回顾性研究：收集内镜中心既往普通白光内镜或共聚焦内镜或病理活检诊断为慢性萎缩性胃炎的病人，回顾性分析并对比普通白光内镜和共聚焦内镜对慢性萎缩性胃炎的诊断价值。 2、前瞻性研究：收集3个月内普通白光内镜下诊断慢性萎缩性胃炎阳性，而病理活检结果为阴性的患者，对其进行共聚焦内镜的实时诊断和事后诊断并进行靶向活检及五处标准部位活检（五处标准部位：胃窦大弯的远端、胃窦小弯的远端 (距幽门口2cm 内)；胃角；胃体大弯中点, 胃角近端约 4cm 处；胃体小弯中点, 距贲门约 8cm 处）。 以五处标准部位活检结果为确诊参照标准，分析共聚焦内镜对慢性萎缩性胃炎诊断的敏感度、特异度、准确度、阳性预测值和阴性预测值，从而评价共聚焦内镜对慢性萎缩性胃炎的诊断价值。 

Description for medicine or protocol of treatment in detail：

1.Retrospective study:collect endoscopy center past patients diagnosed with chronic atrophic gastritis by ordinary endoscopy or confocal laser endomicroscopy or biopsy,to retrospective analysis and compare diagnostic value of ordinary endoscopy and confocal laser endomicroscopy in chronic atrophic gastritis. 2.Prospective study:collect patients with chronic atrophic gastritis,diagnosed positive by ordinary endoscopy and negative by biopsy in nearly 3 months,then ues confocal laser endomicroscopy to real-time-diagnose and post-diagnose,meanwhile to targeted biopsy and biopsy in five standard sites.(five standard sites:large curvature of the antrum and lesser curvature of the antrum 2cm from the pylorus,stomach angle,middle of the gastric body's large curvature 4cm from the stomach angle,middle of the gastric body's lesser curvature 8cm from the gastric cardia) take five standard parts of the biopsy results as the diagnosis reference standard,analysis the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of confocal laser endomicroscopy in the diagnosis of chronic atrophic gastritis,to assess the diagnostic value of confocal laser endomicroscopy in the chronic atrophic gastritis. 

纳入标准：

1、年龄 18-80 岁，可以耐受消化内镜检查；
2、近3个月内普通白光内镜下诊断慢性萎缩性胃炎阳性，而病理活检结果为阴性；
3、自愿加入实验并签署书面知情同意书。

Inclusion criteria

1.18-80 years old, can endure digestive endoscopy;
2.patients with chronic atrophic gastritis,diagnosed positive by ordinary endoscopy and negative by biopsy in nearly 3 months;
3.voluntarily join the experiment and sign a written informed consent.

排除标准：

1、胃切除术病史、急性消化道出血病史、进展期胃癌及其它消化道恶性肿瘤患者
2、患有不能耐受胃镜检查的疾病，包括凝血功能障碍、肾功能不全、严重的心肺疾病、荧光素钠过敏者
3、妊娠及哺乳期妇女
4、无法律行为能力者，医学或伦理学原因影响研究继续进行者

Exclusion criteria：

1.patients with gastrectomy history,acute gastrointestinal bleeding history,advanced gastric cancer and other gastrointestinal malignancies;
2.patients suffering from disease can't tolerate gastroscopy,like coagulation dysfunction,renal insufficiency,serious cardiopulmonary disease and sodium fluorescein allergy;
3.pregnant and lactating women;
4.no legal capacity or having medical and ethical reasons affect the study continues.

The case record form is designed by study leader and study team researchers.The paper version data of all subjects will be locked in the the restricted area of Wuhan Concord Hospital safely,and only test-related researchers can refer to.The electronic version date of all subjects will be entered in a password required secure database,and only lab-related personnel can log in.All test-related data will be kept for at least five years after the test is completed.
