研究目的：

1.探讨拔除气管导管后早期在麻醉恢复室内进行呼吸锻炼对老年人行上腹部开放手术术后肺不张的影响，强调呼吸锻炼的时机对术后肺不张的影响。 2.探讨麻醉恢复室内呼吸锻炼对术后肺部并发症的影响，为拔管后早期呼吸锻炼的意义提供高级别的循证医学证据。  

Objectives of Study：

1. To explore the effect of early breathing exercises in the postanesthesia care unit after extubation on the atelectasis of the elderly undergoing open upper abdomen surgery, and emphasize the influence of the timing of breathing exercises on postoperative atelectasis. 2. To explore the effect of early breathing exercises in the postanesthesia care unit on postoperative pulmonary complications, and to provide high-level evidence-based medical evidence for the significance of early breathing exercises after extubation.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.拟在气管插管全身麻醉下行择期上腹部手术患者；
2.麻醉时间 > 3h；
3.美国麻醉医师协会健康状态分级Ⅰ-Ⅲ。

Inclusion criteria

1. Patients undergoing elective upper abdominal surgery under general anesthesia;
2. Anesthesia time over 3h;
3. With the American Society of Anesthesiology physical status classification of Ⅰ-Ⅲ.

排除标准：

1. 肥胖BMI >= 28kg/m2；
2. 中重度贫血（Hb<90g/L）；
3. 严重的肾脏疾病（肾小球滤过率＜50 ml/min）；
4. 严重的肝脏疾病（Child-Pugh B或C级）；
5. 心肌梗死、心力衰竭和呼吸衰竭病史；
6. 阻塞性睡眠呼吸暂停低通气综合征（OSAHS）；
7. 自发性气胸病史；
8. 神经肌肉疾病；
9. 1个月内的呼吸道感染；
10. 认知功能障碍；
11. 硬膜外阻滞禁忌证；
12. 未获得知情同意。

Exclusion criteria：

1. Obesity BMI >= 28kg/m2;
2. Moderate to severe anemia (Hb<90g/L);
3. Severe kidney disease (glomerular filtration rate < 50 mL /min);
4. Severe liver disease (Child-Pugh B or C);
5. History of myocardial infarction, heart failure, and respiratory failure;
6. Obstructive sleep apnea-hypopnea syndrome (OSAHS);
7. History of spontaneous pneumothorax;
8. Neuromuscular diseases;
9. Respiratory infections within 1 month;
10. Cognitive dysfunction;
11. Contraindications of epidural block;
12. Failure to obtain informed consent.

本研究为单盲设计。参与麻醉过程的麻醉医生以及外科医生团队、参与受试者招募和结局指标评估的研究人员对试验分组均不知情。患者本人、术前访视的研究人员对试验分组知情，术前访视的研究人员将不再参与后续麻醉及评估过程。

Blinding：

This study is a single-blind design. The anesthesiologists and the team of surgeons involved in the anesthesia process, and the researchers involved in the recruitment of subjects and the evaluation of outcome indicators were not aware of the trial groupings. The patients themselves and the researchers of the preoperative visit are informed of the trial groups, and the researchers of the preoperative visit will no longer participate in the follow-up anesthesia and evaluation process.
