研究目的：

本研究在结直肠癌围手术期采用国际上最先进的FTS策略并且加以改进，创新性的引入心理干预，探索结直肠癌围手术期FTS工作模式，目的是为了改善结直肠癌患者术后的生理和心理状况、降低并发症的发生率、提高患者的生活质量、减少住院费用和增加床位周转率。  

Objectives of Study：

Based on the modified fast track surgery of colorectal cancer and mental intervention, the present study aims to improve the physiological and mental status of colorectal cancer patients after operation, reduce the rate of complications, improve quality of life of these patients, decrease in hospital time and in hospital cost, increase bed conversion.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.已诊断为结直肠癌患者，需要手术治疗；2.年龄在18-75岁之间；3.接受标准的结直肠切除、吻合或造瘘；4.近期无化疗史；5.近期无放疗史；6.患者及家属同意FTS治疗。

Inclusion criteria

1. Diagnosed as colorectal cancer and need operation;
2. Between 18 and 75 years old;
3. Undergo standard radical colorectal cancer resection;
4. No recent chemotherapy history;
5. No recent radiotherapy history;
6. The patient and family members agree with fast track surgery treatment.

排除标准：

1.怀孕中或正在哺乳的受试者；2.完全性胃肠梗阻；3.姑息手术或急诊手术；4.需要联合切除脾或胰腺；5.术中出现严重不良事件（指心脑血管意外或危及生命的大出血等）；6.近期接受过针对本病的化疗或放疗；7.严重的心肺疾病；8.HIV感染或存在AIDS相关疾病，或严重的急慢性疾病；9.研究者认为依从性差。

Exclusion criteria：

1. Pregnancy or breast-feeding;
2. Complete gastrointestinal obstruction;
3. Palliative operation or emergency operation;
4. Combined with spleen or pancreas resection;
5. Severe adverse events (cerebrovascular accident, massive hemorrhage, etc.);
6. With recent chemotherapy or radiotherapy history;
7. Severe heart or pulmonary disease;
8. HIV infection, AIDS related diseases or severe acute and chronic disease;
9. Poor compliance of patient.

传统处理组: 无心理干预，常规进行肠道准备，术前12h禁食水，术中不保温，术中不控制液体输入量，常规放置腹腔引流管，术后使用镇痛泵，根据患者情况下床活动，肠道通气后拔除胃管和进食水，术后3-4天拔除尿管，术前预防性使用抗生素一次，术后使用抗生素3-4天

Without mental intervention, bowel preparation routinely, fasting 12 hours before operation, no thermal insulation during operation, without controlling of infusion volume during operation, placing abdominal drainage tube routinely, using analgesia pump after operation, getting out of bed according to patient's situation, pulling out the gastric tube and Ingestion of water and food after recovery of intestinal function, pulling out the catheter 3 to 4 days, use antibiotics 3-4days

FTS处理组: 心理干预，无肠道准备，术前12h口服10%葡萄糖1000mL，术前3h口服10%葡萄糖400mL，术中保温，术中控制液体输入量，不常规放置腹腔引流管，关腹和术后24h用0.5%罗哌卡因20-40mL伤口浸润，口服塞来昔布200mg，每日二次，连续3天，手术当晚离开病床1-2h，术后24h后每天离开病床不少于8小时，术后当天拔出胃管，术后第6h开始口服流质碳水化合物500mL，术后24h开始口服流质碳水化合物2500-300mL，术后当天拔除尿管，术前术后各使用一次抗生素

Mental intervention, without bowel preparation, drinking 1000 mL of 10% glucose 12 hours before operation, drinking 400 mL of 10% glucose 3 hours before operation, controlling of infusion volume during operation, placing abdominal drainage tube occasionally, abdominal incision infiltration with 20 to 40 mL of 0.5% ropivacaine immediately and 24 hours after operation, taking 200mg of celecoxib orally two times per day for 3 days, getting out of bed for 1 to 2 hours at night on the operation day,
