研究目的：

本研究对误吸发生风险较高的老年重症吸入性肺炎（aspiration pneumonia，AP）患者进行改良式心肺康复训练，通过测定胸部HRCT，肺功能，血清炎性因子，老年营养风险评分及生活质量评分，分别从形态学，炎性因子，功能学及生活质量等多方面综合评估治疗效果及复发率，同时测定Th17/Treg平衡的动态变化及IL-17，IL-10和TGF-β等效应细胞因子的表达，并探讨其与重症肺炎严重程度各项指标的相关性，进而评估细胞免疫状态在重症肺炎病程中的演变情况，从免疫学角度为老年吸入性肺炎的治疗提供新的思路和依据。  

Objectives of Study：

In this study, the elderly patients with severe aspiration pneumonia (AP) with high risk of aspiration were trained with modified cardiopulmonary rehabilitation. By measuring chest HRCT, lung function, serum inflammatory factors, geriatric nutritional risk score and quality of life score, morphological, inflammatory factors, Meanwhile, dynamic changes of Th17/Treg balance and expression of effector cytokines such as IL-17, IL-10 and TGF-β were determined, and their correlation with various indicators of severity of severe pneumonia was discussed, so as to evaluate the evolution of cellular immune status in the course of severe pneumonia. From the perspective of immunology, it provides a new idea and basis for the treatment of aspiration pneumonia in the elderly.

纳入标准：

①吸入性肺炎诊断符合2010年日本吸入性肺炎专家共识中的诊断标准：⑴有误吸危险因素者出现发热、咳嗽、咳痰、气短、心动过速等临床表现者；⑵典型影像学表现：重力依赖性肺段渗出影；⑶有明确的误吸现象。②肺炎严重指数（PSI）评分≥ 91 分，即分级为IV-V级的重症肺炎。③洼田饮水试验3级以上患者。④年龄大于60岁。⑤患者能够并愿意配合心肺康复。

Inclusion criteria

① The diagnosis of aspiration pneumonia meets the diagnostic criteria of the consensus of experts on aspiration pneumonia in Japan in 2010: (1) Patients with risk factors of aspiration present fever, cough, expectoration, shortness of breath, tachycardia and other clinical manifestations; (2) Typical imaging findings: gravity-dependent pulmonary exudation; (3) There was definite aspiration. ② Pneumonia severity index (PSI) score ≥ 91 was classified as IV-V severe pneumonia. ③ Patients above Grade 3 in the drinking water test of depression field. ④ Over 60 years old. ⑤ Patients are able and willing to cooperate with cardiopulmonary rehabilitation.

排除标准：

①合并慢性阻塞性肺病、支气管哮喘、肺癌、肺结核等其他呼吸系统疾病；②合并其他系统严重疾病；③存在认知、意识障碍或精神疾病者；④预期寿命不足半年者；⑤治疗依从性较差者；⑥中途脱落不能随访者。

Exclusion criteria：

(1) complicated with chronic obstructive pulmonary disease, bronchial asthma, lung cancer, tuberculosis and other respiratory diseases; ② Complicated with other serious diseases of the system; ③ People with cognitive or consciousness disorders or mental diseases; (4) Life expectancy less than half a year; (5) Poor treatment compliance; ⑥ Can not fall off with visitors.
